摘　　要：目的考察右美托咪定在术前甲胎蛋白(AFP)高水平患者行肝脏肿瘤射频消融术(RFA)的监护麻醉效果。方法将52例原发性肝癌行肝脏肿瘤RFA患者,根据是否采用右美托咪定分为对照组25例,观察组27例。结果两组麻醉前的血压、心率、清醒程度和T淋巴细胞亚群无明显差异(P>0.05);麻醉后,与对照组比较,观察组注药后10 min(T1)和注药后30 min(T2)的血压和心率较低(P<0.05),其余时间点的血压和心率无明显差异,CD3+细胞、CD4+细胞和CD4+/CD8+显著降低(P<0.05),两组麻醉深度无显著性差异(P>0.05)。两组经RFA治疗后无死亡病例,与对照组比较,观察组术后的疼痛、恶心/呕吐例数较少(P<0.05),麻醉满意例数较多(P<0.05),治疗时间和苏醒时间较短(P<0.05),术后AFP较低(P<0.05)。结论对于接受RFA治疗的AFP高水平肝癌患者,应用右美托咪定麻醉,安全性高,可显著改善患者的细胞免疫状态,有利于预后。Objective To investigate the application of dexmedetomidine （Dex） in high levels of alpha-fetoprotein （AFP）patients who underwent liver tumor radio frequency ablation. Methods 52 cases were divided into 2 groups depending on whether use of Dex： 25 cases in the control group, 27 cases in the observation group. Results Before anesthesia, blood pressure, heart rate, alertness and T lymphocyte subsets in 2 groups had no differences （P 〉 0. 05）. After anesthesia, compared with the control group, blood pressure and heart rate of T1 and T2 in the observation group had decreased （P 〈 0.05）. The rest of the time points, blood pressure and heart rate in 2 groups had no difference. CD3^＋ cells, CD4^＋ cells and CD4^＋/CD8^＋ had significantly decreased （P 〈 0. 05）. The anesthesia depth of 2 groups had no significant differences （P 〉 0. 05）. There had no deaths after operation. Compared with the control group, the cases of pain, nausea/vomiting after operation had decreased（ P 〈 0. 05）, the cases of satisfaction had increased（P 〈 0. 05）, treatment time and recovery time had decreased（P 〈 0.05）, AFP had decreased in the observation group（P 〈 0. 05）. Conclusion Hepatocellular carcino- ma patients with higher RFA undergo liver tumor radio frequency ablation should accept Dex anesthesia, which is safe and significantly be good for cellular immune status and prognosis.
