枯草杆菌二联活菌肠溶胶囊与四联疗法治疗老年人幽门螺旋杆菌感染临床观察

摘　　要：目的观察枯草杆菌二联活菌肠溶胶囊(制剂)与传统四联疗法治疗老年人幽门螺杆菌感染的安全性及临床效果,寻找较为适合老年人的根除幽门螺杆菌的方案。方法对14C呼气试验阳性的老年(≥60岁)患者224例随机分为2组,观察组受试者每人口服雷贝拉唑钠肠溶胶囊20 mg(1次/d)、复方铝酸铋1.3 g(3次/d)、阿莫西林胶囊0.5 g(2次/d)、克拉霉素0.5 g(2次/d)和2 h后服用枯草杆菌二联活菌制剂2粒(3次/d),治疗2周;对照组受试者每人除不服用枯草杆菌二联活菌制剂外,与观察组服药相同,治疗2周。结果观察组有效率为7 8.90%(86/109),对照组有效率为81.25%(91/112),两组间差异无统计学意义(χ2=0.20,P>0.05)。治疗结果显示两种方案对根治老年人幽门螺杆菌感染均获得了良好的效果。在不良反应方面,观察组总不良反应率为20.54%;对照组总不良反应率为为49.11%,观察组的不良反应率显著低于对照组,其差异具有统计学意义(P<0.05)。结论实验所用两种方案治疗老年人幽门螺杆菌感染均有良好的效果,而枯草杆菌二联活菌制剂联合四联疗法治疗幽门螺杆菌感染可以减少不良反应,提高治疗的安全性。Objective To observe safety and therapeutic effect in treatment of Helicobacter pylori （ Hp） infection by using of enteric capsules of bigeminy living bacillus subtilis （ Medilac ） in combination with classical quadruple therapy, and to explore a more appropriate treatment in curing of Hp infection in the elderly.Methods A total of 224 patients （≥60 years age） were randomly divided into two groups based on Hp positive reaction by the 14 C breath test, subject in observa-tion group was orally given in 20 mg pabeprazole sodium enteric-coated capsules （ once per day） , 1.3 g bismuth aluminate （three times per day）, 0.5 g amoxicillin （twice per day）, 0.5 g clarithromycin （twice per day） and two capsules of Medi-lac （ three times per day） , for two weeks;subject in control group was orally given the same medicines as the observation group without taking Medilac, for two weeks.Results The observation and control group obtained an efficiency of 78.90%（86 /109） and 81.25% （91 /112）, respectively.There was no statistical difference between the two groups （χ2=0.20, P〉0.05）.These results suggested that the two treatments had a good effectiveness in curing of Hp infection in the elderly.However, in the aspect of adverse reaction, the occurrence rate （20.54%）in the observation group was obvi-ously lower than that （49.11%） in the control group, it showed a statistical significance （ P〈0.05） in two groups.Con-clusion The two selected treatments had a favorable effectiveness in treatment of Hp infection in the elderly.The combi-nation of Medilac and quadruple therapy could reduce adverse reaction and improve the safety in curing of Hp infection.
