舒芬太尼复合酮咯酸氨丁三醇用于骨科手术后自控镇痛的临床观察

摘　　要：目的观察舒芬太尼复合酮咯酸氨丁三醇用于骨科患者手术后自控静脉镇痛的效果。方法选择择期骨科下肢手术患者80例,随机分为2组,每组40例。舒芬太尼组(Ⅰ组):舒芬太尼3.0μg/kg+昂丹司琼4 mg;舒芬太尼复合酮咯酸氨丁三醇组(Ⅱ组):舒芬太尼2.0μg/kg+酮咯酸氨丁三醇2.0 mg/kg+昂丹司琼4 mg。两组均用生理盐水稀释到100 m L,持续输注速度2 m L/h,追加0.5 m L/次,追加间隔时间15 min。观察术后疼痛的视觉模拟评分(Visual Analogue Scale,VAS)及不良反应发生情况。结果两组患者在术后各时点VAS评分组间比较差异无统计学意义(P>0.05),镇痛效果满意度Ⅱ组高于Ⅰ组(P<0.05),不良反应发生率Ⅱ组低于Ⅰ组(P<0.05)。结论舒芬太尼复合酮咯酸氨丁三醇用于骨科患者手术后自控静脉镇痛,镇痛效果确切,患者满意度高,不良反应少。Objective To investigate the effect of sulfentanil combined with ketorolac tromethamine for post-operative patient-controlled intravenous analgesia （ PCIA） in the orthopedic surgery. Methods Eighty cases of patients undergoing lower limb operation were randomly divided into two groups：sulfentanil group （ groupⅠ,n=40 ） and sul-fentanil plus ketorolac tromethamine group（groupⅡ,n=40）. Both groups received PCIA after operation. PCIA solu-tion contained sulfentanil 3 μg/kg and ondansetron 4 mg in 100 mL of normal saline in group Ⅰ. PCIA solution con-tained sulfentanil 2. 0 μg/kg,ketorolac tromethamine 2. 0 mg/kg and ondansetron 4 mg in 100 mL of normal saline in group Ⅱ. Both PCIA pumps were set up with a 0.5 mL bolus dose,a 15 min lockout interval and background infusion at a rate of 2 mL/h. The visual analogue scale （ VAS ） and adverse effects were recorded. Results No significant difference was observed in the VAS between the two groups at mentioned time after operation （ P〉0.05 ） . Compared with group Ⅰ,the number of satisfied demand was significantly higher in group Ⅱ than that in group Ⅰ （P〈0.05）, the incidence of adverse reaction was significantly decreased in group Ⅱ （P〈0.05）. Conclusion Administration of sulfentanil combined with ketorolac tromethamine has an exact effect on PCIA in the orthopedic surgery with high satis-factory degree and little adverse effect.
