摘　　要：目的介绍第一掌背动脉远端支皮瓣在拇指末节皮肤软组织套状缺损中的应用。方法2013年1月至2014年5月，对34例拇指末节皮肤软组织套状缺损的患者，采用第一掌背动脉远端支皮瓣进行修复。结果除2例皮瓣在术后24～72h内出现轻度肿胀，行对症处理后肿胀消退，其余皮瓣均顺利存活。未见皮瓣血供不足及感染等并发症，所有创口均l期愈合。术后随访时间为6-18个月，皮瓣质地柔软，外观恢复满意，静态两点分辨觉为6．0—8．5mm，平均6．7mm。供区外观良好，供指关节活动无明显受限。结论第一掌背动脉远端支皮瓣为拇指末节皮肤软组织套状缺损提供了一种良好的手术方式选择。Objective To introduce the Climcal application of first dorsal metacarpal distal artery flap in reconstruction of thumb-tip degloving injuries. Methods From January 2013 to May 2014, 34 cases of thumb- tip degloving injury were treated with the first dorsal metacarpal distal artery flap. Results Mild swelling of the flap occurred in 2 cases within 24 to 72 hours postoperatively. Conservative treaunent alleviated the congestion. All flaps survived completely. There were no ischemia or infection of the flaps. Primary wound healing was achieved in all cases. All the 34 patients had satisfactory motion and sensory function after 6 to 18 months of follow-up. Flap texture was soft. Static two-point discrimination was 6.7 mm （range, 6.0 to 8.5 n^n）. Donor site appearance was acceptable. There was no impairment to donor finger joint motion. Conclusion The first dorsal metacarpal distal artery flap provides a good option for thumb-tip degloving iniury reconstruction.
