摘  要:目的探讨白首乌C21甾苷对慢性心力衰竭(CHF)大鼠新生血管形成及胰岛素样生长因子-1(IGF-1)表达的影响,探讨其防治CHF的可能作用机制。方法采用左冠状动脉前降支结扎术建立Wistar大鼠CHF模型,随机分为两组:模型组(n=8)给予等体积生理盐水;白首乌C21甾苷组(n=9)给予白首乌C21甾苷1.0 g/kg。另设假手术组(n=10),给予等体积生理盐水。3周后进行左心室顺应性测量,并分别采用苏木精-伊红(HE)染色和流式细胞术测量微血管密度(MVD)和CD34+细胞阳性率。进一步采用Western blot和Real-Time PCR分析检测IGF-1蛋白和IGF-1 m RNA表达的变化。结果与模型组比较,白首乌C21甾苷组的左心室顺应性明显改善,MVD和CD34+细胞阳性率升高,差异有统计学意义(P<0.05)。与模型组比较,白首乌C21甾苷组的IGF-1蛋白和IGF-1m RNA的表达显著增强,差异有统计学意义(P<0.05)。结论白首乌C21甾苷可有效改善CHF大鼠左心室顺应性,对衰竭心肌有明显的保护作用,可通过IGF-1诱导CD34+细胞动员至外周血促进新生血管的形成。Objective To investigate the effect of C21 steroidal glycoside of Cynanchum Auriculatum on angiogenesis and expression of insulin-like growth factor-1 (IGF-1) in chronic heart failure (CHF) rats and discuss its possible mechanism. Methods Anterior descending branch of left coronary artery was ligated to set up CHF Wistar rats model and rats were randomly divided into two groups: model group (n = 8) was administered equal volume of normal saline; C21 steroidal glycoside of Cynanchum Auriculatum group (n = 9) was administered orally C21 steroidal glycoside of Cy- nanchum Aurieulatum according to 1.0 g/kg. Another sham group (n = 10) was administered equal volume of normal saline orally. After 3 weeks, left ventricular compliance was assessed. Microvessel density (MVD) and the positive rate of CD34^+ cell was measured by HE staining and flow cytometry respectively. Further Western blot and Real-Time PCR were used to detect the protein and mRNA expression of IGF-1. Results Compared with model group, left ventricular compliance in C21 steroidal glycoside of Cynanchum Auriculatum group was improved significantly, MVD and the positive rate of CD34^+ cell were elevated, the differences were statistically significant (P 〈 0.05). Compared with model group, IGF-1 protein and the expression of IGF-1 mRNA were enhanced significantly, the differences were statistically significant (P 〈 0.05). Conclusion C21 steroidal glycoside of Cynanchum Auriculatum has obvious protective effect on heart failure by improving effectively left ventricular compliance and promote the formation of neovascularization through inducing CD34^+ cell mobilization to the outside blood by IGF-1.
