摘  要:目的 探讨体外培养1 d对玻璃化解冻卵裂期胚胎移植效果的影响。方法 选择本中心2012年1~7月进行玻璃化冷冻并复苏的277周期卵裂期胚胎作为研究对象,根据解冻后是否多培养1 d分为对照组和实验组。对照组在解冻当天移植,共123周期。实验组解冻后多培养1 d,共154周期。实验组又分为培养后含桑葚胚组(A组)及无桑葚胚组(B组)。观察延长体外培养时间对妊娠结局的影响以及培养后胚胎发育情况与妊娠结局的关系。结果 实验组的妊娠率和着床率显著高于对照组,差异有统计学意义(P〈0.05)。A组的妊娠率与着床率显著高于B组,差异有统计学意义(P〈0.05)。结论 体外培养1 d不影响玻璃化解冻后卵裂期胚胎的移植效果,延长培养时间有利于选择发育良好的胚胎并显著提高临床妊娠率和着床率。Objective To explore the influence of one-day culture in vitro on embryo transplantation effect of vitrification-unfreezing at cleavage stage. Methods 277 cycles of cleavage stage embryos on revived transplantation of vitrification-unfreezing at cleavage stage from January 2012 to July in our center were selected and divided into the control group (123 cycles) and the experimental group (154 cycles) according to whether one-day more culture after unfreezing. The control group was transplantated during the same day after unfreezing,the experimental group was transplantated one-day more culture after unfreezing.The experimental group was again divided into morula group (group A) and non- morula group (group B) after cultivation.Influences on extended culture in vitro on pregnancy outcome and relationships between embryonic development after cultivation and pregnancy outcome were observed. Results The pregnancy rate and the rate of embryonic implantation in the experimental group was higher than that in the control group,with signifi- cant difference (P〈0.05).The pregnancy rate and the rate of embryonic implantation in group A was higher than that in group B,with significant difference(P〈0.05). Conclusion One-day more culture in vitro does not affect on embryo transplantation of vitrification-unfreezing at cleavage stage.Extension of culture period is beneficial to select well-developed embryo and also remarkably improves clinical pregnancy rate and rate of embryonic implantation.
