摘  要:目的:初步探讨尿液中蛋白的差异与早期糖尿病肾病不同中医证型之间的相关性。方法:对符合纳入标准的150例早期糖尿病肾病患者按中医辨证分型分为肝肾阴虚证组、气阴两虚证组和脾肾阳虚证组。用双向凝胶电泳(2-DE)结合质谱分析方法对3组患者尿蛋白进行比较蛋白质组学分析。结果:在肝肾阴虚证高表达的蛋白质有:Nogo蛋白、单克隆抗体Ig M。在气阴两虚证高表达的有:动力蛋白、转铁蛋白、Ig Kappa链、和两种白蛋白。脾肾阳虚证高表达的蛋白质有:白蛋白(分子量52 047.8)、转甲状腺素蛋白、αl-抗胰蛋白酶。结论:这些差异蛋白与早期糖尿病肾病不同证候的形成有关,也为进一步研究证候实质提供依据。Objective:To preliminarily study the relationship between the types of protein in the urine and the different syndromes of diabetic nephropathy in early stage. Methods : 150 patients who met the inclusion criteria of diabetic nephropathy were divided into each syndrome type according to the syndromes of TCM : spleen and kidney Yin deficiency syndrome, syndrome of deficiency of both Qi and Yin, spleen and kidney Yang deficiency syndrome. The proteins in the urine among the three groups were analyzed using two dimensional gel electrophoresis (2 -DE)and mass spectrometry. Results: Nego and IgM were found in the high protein expression of the spleen and kidney Yin deficiency syndrome. Dynamin, Transferrin, IgKappa and two types of albumin were found in the high protein expression of syndrome of deficiency of both Qi and Yin. Albumin(52 047.8Da) ,Transthyretin and α1 - antitrypin were found in the high protein expression of spleen and kidney Yang deficiency syndrome. Conclusion : These differential proteins are involved in the creation of different syndromes of diabetic nephropathy in early stage, providing the evidence for the further studies on the essence of syndrome.
