摘  要:目的探讨微创钢板治疗肱骨干骨折的并发症相关因素及其对策。方法回顾性分析我院2008年1月至2014年5月收治的肱骨干骨折行微创钢板手术治疗166例患者资料,对可能影响术后并发症发生的各因素进行单因素分析以及多因素Logistic回归分析,并提出相应的对策。结果从单因素分析结果可知,并发症患者与无并发症患者比较,在年龄、营养状况、合并其他内科疾病、原发骨折类型上具有统计学意义(P<0.05);而在受伤到手术时间、手术时间、性别、骨折部位、麻醉方式上两组患者不具有统计学意义(P>0.05)。多因素分析结果显示,营养状况、合并其他内科疾病、骨折类型以及年龄进入回归模型(P<0.05),是影响微创钢板治疗肱骨干骨折患者术后并发症发生的独立危险因素。结论微创钢板治疗肱骨干骨折术后并发症发生可能与患者营养状况、合并其他内科疾病、骨折类型以及年龄有关,在治疗中除了注意上述危险因素外,还需注意规范手术操作、术后正确功能锻炼,以尽可能减少术后并发症的发生。Objective To explore the related factors of complications of minimally invasive percutaneous plate in treatment of humeral shaft fracture and its countermeasures. Methods A retrospective analysis was performed for minimally invasive plate of surgical treatment of 166 cases of humeral shaft in our hospital between January 2008 and May 2014 in our hospital. The possible influence of the postoperative complications of the various factors of single factor analysis and multi factor Logistic were analyzed, and the corresponding countermeasures were proposed. Results The results from single factor analysis showed there was signigicant difference between patients with and without complications,in age, nutritional status, combined with other diseases of internal medicine, primary fracture types ( P 〈 0.05 ) ; while in the injury to operation time, operation time, gender, fracture position, anesthesia between the two groups of patients, there was no statistical significant difference ( P 〉 0.05 ). Multi factor analysis showed that the nutritional status, combined with other diseases of internal disease and age entered the regres- sion model ( P 〈 0.05 ) ,independent risk factors for the effect of minimally invasive plate in treatment of humeral shaft frac- tures in patients with postoperative complications. Conclusion Postoperative complications may be associated with nutritional status, internal disease and advanced age during the treatment of minimally invasive plate for humeral shaft fracture. We should pay attention to these risk factors, but also need to pay attention to the correct function exercise on the operation specification, postoperative, taking place as much as possible to reduce the postoperative complications.
