代谢综合征患者血清抵抗素瘦素与肥胖关系的探讨

摘　　要：目的探讨代谢综合征(MS)患者抵抗素、瘦素和肥胖的关系。方法根据MS和肥胖的诊断标准,从健康体检人群中选取肥胖MS 31例,非肥胖MS 26例,健康对照30名,采用放射免疫方法检测血清抵抗素、瘦素,同时检测身高、体质量、血压、空腹血糖、血脂、血尿酸、胰岛素,并计算体质量指数和胰岛素抵抗指数。结果肥胖与非肥胖MS组和健康对照组的收缩压、空腹血糖、高密度脂蛋白胆固醇、甘油三酯、瘦素、抵抗素、胰岛素和胰岛素抵抗指数相比差异有统计学意义(P<0.05)。肥胖MS征组与非肥胖MS组的空腹血糖、瘦素和胰岛素相比差异有统计学意义(P<0.05)。抵抗素水平的升高与体质量指数、血尿酸、收缩压和胰岛素抵抗指数相关,瘦素水平的升高与体质量指数、空腹血糖、胰岛素和胰岛素抵抗指数相关。结论血清瘦素和抵抗素水平与MS密切相关,高血清瘦素和抵抗素在MS和肥胖的形成与发展中起重要作用。Objective In order to elucidate the relationship between resistin ,leptin and obesity in patients with metabolic syndrome.Methods We examined leptin and resistin in patients with and without metabolic syn‐drome .According to the diagnostic criterion of metabolic syndrome and obesity ,we selected 31 patients with met‐abolic syndrome and obesity .We also selected 26 patients with metabolic syndrome but without obesity .There are 30 patients without metabolic syndrome .At the same time ,their weight ,height ,blood pressure ,blood glucose , blood lipid ,Uric acid（UA） ,insulin ,body mass index（BMI） and insulin resistance index were measured.Results There are significant differences in systolic pressure ,blood glucose ,HDL‐C ,TG ,leptin ,resistin ,insulin and in‐sulin resisitance index between patients with metabolic syndrome and patients without metabolic syndrome （ P 〈0.05） .There is no significant differences in age ,diastolic pressure ,BMI ,TC ,LDL‐C ,and UA between patients with metabolic syndrome and patients without metabolic syndrome （ P〉0.05） .There are significant differences in blood glucose ,leptin and insulin between patients with metabolic syndrome and obesity and patients with metabol‐ic syndrome but without obesity（ P〈0.05） .There is no significant differences in age ,diastolic pressure ,systolic pressure ,BMI ,TC ,TG ,HDL ,LDL ,UA ,resistin and insulin resistance index between patients with metabolic syndrome and obesity and patients with metabolic syndrome but without obesity （ P 〉0.05） .The increase of re‐sistin is associated with BMI ,UA ,systolic pressure and insulin resistance index .The increase of leptin is associ‐ated with BMI ,blood glucose ,blood insulin and insulin resistance index .Conclusion Serum leptin and resistin are associated with metabolic syndrome .High serum leptin and resistin play important roles in the development of metabolic syndrome and obesity.
