摘 要:目的探讨饮水联合低管电流扫描技术对CT尿路造影(CTU)显示尿路的影响。方法 120例患者按照检查顺序随机分为常规管电流饮水组(A组)、常规管电流不饮水组(B组)、低管电流饮水组(C组)和低管电流不饮水组(D组),A、C组扫描前40 min饮水10 ml/kg体重。在CTU分泌期图像上将尿路分为4段:肾内集合系统S1、近段S2(输尿管起始部至腰大肌跨越处)、中段S3(从腰大肌跨越处至髂血管处)、远段S4(从髂血管处到膀胱),分别比较四组各段尿路腔内平均CT值、各段尿路充盈情况评分、图像质量评分和辐射剂量的差异。结果四组各段尿路结构CT值差异均无统计学意义(P>0.05);A组与B组各段充盈评分差异均有统计学意义(P<0.05),C组与D组各段充盈评分差异均有统计学意义(P<0.05),饮水组尿路充盈情况评分高于不饮水组(P<0.05);四组各段尿路图像质量评分之间差异无统计学意义(P>0.05);低管电流与常规管电流组间CT剂量指数(CTDIvol)、剂量长度乘积(DLP)和有效剂量(ED)差异均有统计学意义,其中低管电流组较常规管电流组ED降低33.92%。结论饮水后CTU低管电流技术可在降低辐射剂量,有效改善尿路充盈情况的同时满足诊断要求。Objective To explore the influence of drinking water with low tube current to display urinary tract structures. Methods The 120 consecutive patients performed the check order and were divided into four groups which in standard dose with drinking water group( A group),in standard dose without drinking water group( B group),in the low dose with drinking water group( C group) and in the low dose without drinking water group( D group). The group A and C drink water 10 ml / kg before scanning. The CTU is divided into four period: S1 is collection systems in kidney,S2 is ureteral recently( ureteral starting to psoas major span),S3 is ureteral middle( from psoas major span to common iliac artery),S4 is far end( from common iliac artery to bladder). The CT value,subjective scoring of image quality and radiation dose between four groups. Results There was no statistical significance difference in the CT value. There was statistical significance difference between the A group and B group,C group and D group of full score. The score of drinking water group is higher than without drinking water. There was no statistical significance difference in the subjective scoring of image quality. There was statistical significance difference between the four groups of CTDIvol,DLP and effective dose. Low dose acquisitions were obtained with average 33. 92% dose reduction. Conclusion CTU low tube current after drinking water can reduce the radiation dose and improve urinary filling effectively and at the same time meet the diagnostic requirements.
