彩色多普勒超声对糖尿病肾病肾脏血管损害的研究

摘　　要：目的：探讨彩色多普勒超声在评价糖尿病肾病肾血管病变中的应用价值。方法：46例糖尿病肾病患者分成2组，彩超下肾脏血流灌注分级，并测量叶间动脉、小叶间动脉的收缩期峰值流速，舒张末期最低流速，阻力指数及搏动指数，与30例正常人对照分析。结果：糖尿病肾病肾脏损害加重，超声显示血流灌注分级增加；糖尿病肾病1组小叶间动脉阻力指数及搏动指数增高；糖尿病肾病2组肾叶间动脉、小叶间动脉的收缩期峰值流速，舒张期最低流速均较正常组减低，阻力指数及搏动指数增高。结论：彩色多普勒超声检测糖尿病肾病肾血流参数的变化对评估糖尿病肾病肾血管床的损害有一定参考价值。Objective：To discuss the renal vascular damage in diabetic nephropathy by color doppler ultrasound.Methods：46 pa-tients of diabetic nephropathy were divided into two groups.Observed the renal blood flow distribution conditions using color doppler ultra-sound, and recorded their blood flow parameters such as peak velocity in systole（ Vmax） , end-diastolic velocity （ Vmin） , resistive index （ RI） and pulsed index（ PI） using pulsed doppler ultrasound and compared with 30 healthy persons.Results：In group 1 the RI and PI of interlobular arteries increased ,in group 2 Vmax,Vmin,RI and PI of interlobar and interlobular arteries both increased.Conclusion：The renal blood flow parameters from color doppler ultrasound may provide a certain valuable information for estimating renal vascular damage in diabetic nephropathy.
