摘  要:目的探讨肱骨头骨密度对肩袖肌腱生物力学的影响。方法 18只家兔随机分为3组:对照组、OVX-Saline组、OVX-PTH组。8周后观察兔肱骨头骨密度(BMD),ISP肌腱止点强度及其组织学变化。结果对照组和OVX-PTH组肱骨头BMD明显高于OVX-Saline组(对照组P=0.000 4,OVX-Saline组P=0.002 4)。3组间断裂应力比较差异无统计学意义(P>0.05),但对照组和OVX-PTH组断裂强度较高,肱骨头BMD与断裂应力呈线性正相关(r2=0.54)。组织学显示OVX-PTH组与OVX-Saline组相比,肌腱止点具有更完整、清晰的结构区域。结论肱骨头骨量丢失降低冈下肌腱止点强度;PTH增加肱骨头BMD并提高冈下肌腱止点的强度。Objective To investigate the effect of bone mineral density of humeral head on biomechanics of rotator cuff.Methods Eighteen rabbits were divided into three groups: control group, OVX-Saline group and OVX-PTH group. Humeral head BMD, ISP tendon insertion strength, and histology were assessed. Results Humeral head BMD of the control and OVXPTH groups were significantly higher than that of OVX-Saline group(P =0.000 4 and P =0.002 4, respectively). No significant difference was found in failure stress among the three groups(P〉0.05), but there was an expected trend with the control group and OVX-PTH group presenting higher failure strength compared to the OVX-Saline group. BMD at the humeral head showed a positive linear correlation with stress(r^2=0.54). Histology results showed a better structural enthesis in the OVXPTH group ISP compared to the OVX-Saline group. Conclusion Bone loss of the humeral head leads to decreased tendon/bone insertion strength of the infraspinatus tendon. PTH administration can increase bone density of the humeral head and may improve the mechanical properties of the infraspinatus tendon enthesis.
