摘 要:目的利用神经电生理检测技术评价硫辛酸治疗糖尿病周围神经病变患者的临床疗效。方法选择2014年2月—2015年2月菏泽市第三人民医院神经内科诊断糖尿病周围神经病变患者110例,按照入院先后顺序,依据随机数字表法,将患者分为观察组与对照组各55例。利用神经电生理检测技术测定正中神经、尺神经、胫神经、腓总神经的运动神经传导速度(MCV)与感觉神经的传导速度(SCV)。比较二组患者临床疗效差异。结果治疗前,二组患者年龄、性别、病程、正中神经、尺神经、胫神经、腓总神经的运动神经传导速度(MCV)与感觉神经的传导速度(SCV)差异无统计学意义(P>0.05),治疗后,观察组与对照组比较,患者正中神经、尺神经、胫神经、腓总神经的运动神经传导速度(MCV)与感觉神经的传导速度(SCV)明显减少,二组比较差异有统计学意义(P<0.05),观察组与对照组患者比较,主诉肢体麻木者7例(12.73%)比16例(29.10%),t=2.984;疼痛1例(1.82%)比5例(9.09%),t=3.051;烧灼感6例(10.91%)比14例(25.45%),t=2.995;肢体无力感者9例(16.36%)比18例(32.73%),t=3.052;均明显减少,二组患者治疗后差异有统计学意义(P<0.05)。结论硫辛酸注射液治疗糖尿病周围神经病变可以明显促进周围神经功能传导,起到加强改善糖尿病周围神经异常症状的临床作用。Objective To investigate the clinical efficacy of alipoic acid in patients with diabetic peripheral neuropathy by the use of electrophysiological evaluation. Methods One hundred and ten patients with diabetic peripheral neuropathy coming from neuropathy department of author's hospitaly from February 2014 to February 2015 were randomly divided into treatment group and control group, fiftyfive cases in each group. The patients in the control group were given conventional internal medicine therapy, while alipoic acid was additionally offered in the treatment group. The nerve conduction velocity of median nerve, ulnar nerve, tibial nerve and peroneal nerve motor nerve and sensory nerve were measured by the use of electrophysiological evaluation. The difference of clinical effects was compared between the two groups. Results After treatment, the nerve conduction velocity of median nerve, ulnar nerve, tibial nerve and peroneal nerve motor nerve and sensory nerve in the treatment group significantly reduced than the control group, there were significant difference. The symptoms of limb numbness, soreness, burning sensation and limb weakness in the treatment group were less than the control group, there were statistical significant difference in two groups after treatment(P〈0. 05). Conclusion The investigation suggests that alipoic acid can accelerate the nerve conduction velocity and strengthen the clinical effect and promote the symptoms in treatment in patients with diabetic peripheral neuropathy.
