儿童传染性单核细胞增多症及相关疾病的肝功能分析

摘　　要：目的分析儿童传染性单核细胞增多症(传单)及类传染性单核细胞增多症(类传单)的常用肝功能指标水平,探讨其在肝功能损伤时的变化特点。方法对2014年7月—2015年6月我院儿科住院的20例传单及32例类传单患儿的临床资料进行回顾性分析,总结丙氨酸氨基转氨酶(ALT)、天门冬氨酸氨基转氨酶(AST)、谷氨酰转移酶(GGT)、乳酸脱氢酶(LDH)、腺苷脱氨酶(ADA)的水平及特点。结果 1.传单较类传单更易出现ALT升高,年龄大者易出现ALT异常,差异有统计学意义(P<0.05),ALT阳性发生率较AST高,差异无统计学意义(P>0.05)。2.GGT与ALT水平呈正相关(r=0.761,P<0.01)。3.急性期LDH和ADA均升高,二者呈正相关(r=0.664,P<0.01)。结论传单及类传单患儿肝功能损伤的原因与机体免疫异常及胆汁淤阻有关。此类疾病中ALT能敏感反映肝功能,GGT异常者肝功能损伤严重;ADA可作为判断患儿免疫功能恢复的指标,需注意随访。Objective To anaIyze Iiver function of infectious mononucIeosis and infectious mono-nucIeosis-Iike disease,to investigate IeveIs, characteristics and possibIe factors of severaI in-dexes of Iiver function in chiIdren infectious mononucIeosis and reIated diseases.Methods AII 52 patients（20 IM,32 IM-Iike） who were admitted to the pediatric ward of our hospitaI from JuIy, 2014 to June,2015 were enroIIed.To evaIuate their Iiver function retrospectiveIy,which incIuding aIanine aminotransferase （ ALT ） , aspartate aminotransferase （ AST ） , actate dehydrogenase （ LDH） ,and adenosine deaminase（ ADA） .Results 1.ALT evaIuation was more prone to occur in IM chiIdren than in IM-Iike,and significantIy occur in eIder chiIdren （ P〈0.05） .In both diseases, the positive rates of ALT were higher than AST.2.The correIation between the IeveI of GGT and ALT had statisticaI significance （r=0.761,P〈0.01）.3.Both of LDH and ADA were eIevated in a-cute phase of IM, and were abnormaI in convaIescence of most of patients.Conclusion The nosogenesis of Iiver function was reIated to dysimmunity and biIe stasis possibIy.The IeveI of ALT was a sensitive indicator to refIect Iiver function damage in IM and IM-Iike chiIdren.Those patients with high IeveI of GGT had more severe hepatic dysfunction.ADA may be used as the indicator of immune function recovery in patients,which need to be foIIowed up.
