我院近5年大肠埃希菌的分布及耐药性分析

摘　　要：目的：调查大肠埃希菌的耐药性变迁，为临床医师用药管理及医院感染控制提供依据。方法采用VITEK 2 compact全自动细菌鉴定仪对2010年—2014年我院分离出的大肠杆菌进行鉴定，采用WHONET5.6及SPSS10.0对数据进行分析。结果2010年—2012年，我院大肠埃希菌在临床中的分离率为15%左右，而2012年—2014年上了一个台阶，稳定在20%左右。阳性率最高的三类标本分别是引流液、分泌物类及尿液；阳性率最高的三个科室分别是重症医学科、神经外科及泌尿外科。产超广谱β-内酰胺酶菌株2010年为43.6%、2011年为55.6%，之后逐年稳步下降至45.2%，略高于2010年。我院检出的大肠埃希菌对氨苄西林的耐药率高达92.0%~96.4%，对头孢类的耐药率也较高，但有下降趋势；对第四代头孢菌素头孢替坦比较敏感，对亚胺培南及厄他培南在近3年内100%敏感。结论我院大肠埃希菌对各种常用药的耐药率虽在5年内有所下降，但仍维持在较高的水平，应加强对细菌耐药性的监控，遏制多药耐药菌的产生与流行。ObjectiveTo investigate the antimicrobial resistance of E.coli strains isolated from patients in our hospital during2010—2014 and provide the evidence for clinical application of antibiotics.MethodsThe VITEK 2 compact was used to do drug susceptitibility test.WHONET5.6 and SPSS10.0 were used to do data analysis.Results During 2010—2014,about 15% E.coli was isolated from the bacterium in our hospital,the rate increased to about 20% during 2012—2014.The sample with the highest three positive rate was drainage liquid,secreta,and urine. The department with the highest three positive rate was ICU,neurosurgery,urinary department. The detection rate of extensive-spectrum beta-lactamase（ESBLs）was 43.6%in 2010,which increase to 55.6% in 2011 and dropped every year to 45.2% in 2014.The resistance rate to ampicillin was 92.0%-to 96.4%.The resistance rate to cephalosparoin was high,but have an drop tendency.The E.coli in our hospital was sensitive to the cefotetan and was 100%sensitive to imipenem and Ertapenem.ConclusionThe resistance rate of E.coli in our hospital had a drop tendency in the recent five years,but still in a high level. We should intensify the monitoring and controlling of antibiotic resistance so as to prevent the multidrug-resistant strains from occurring and prevailing.
