加巴喷丁联合神经阻滞治疗颈源性头痛的临床效果

摘　　要：目的 探讨加巴喷丁联合神经阻滞治疗颈源性头痛的临床效果.方法 选取2012年6月至2013年4月河北省衡水市哈励逊国际和平医院疼痛科诊治的颈源性头痛患者60例,完全随机分为观察组和对照组,各30例.对照组给予神经阻滞治疗,观察组采用加巴喷丁联合神经阻滞治疗.在治疗前、治疗后1周、1个月、3个月时,采用疼痛视觉模拟量表（VAS）评分和睡眠质量（QS）评分评估2组患者治疗效果,同时观察患者的不良反应发生情况.结果 观察组和对照组患者治疗后1周、1个月、3个月的VAS和QS评分均明显低于治疗前[观察组：（1.5±0.6）、（0.6±0.5）、（1.8±0.7）分比（6.8±2.5）分,（1.9±0.2）、（1.1±0.1）、（0.9±0.2）分比（4.5±0.4）分;对照组：（1.7±1.2）、（1.1±0.6）、（3.3±1.3）分比（6.7±2.8）分,（3.6±0.3）、（2.7±0.4）、（2.5±0.6）分比（4.6±0.3）分],且治疗后1个月和3个月观察组VAS及QS评分、治疗后1周观察组QS评分明显低于对照组,差异均有统计学意义（均P＜0.05）.治疗后3个月,观察组治愈率和总有效率明显高于对照组[73.3％ （22/30）比53.3％ （16/30）,100.0％ （30/30）比90.0％ （27/30）],差异有统计学意义（P＜0.05）.2组均无严重不良反应发生.结论 加巴喷丁联合神经阻滞治疗颈源性头痛安全有效,其疗效优于单一的神经阻滞治疗.Objective To evaluate the clinical effects of gabapentin combined with nerve block on cervicogenic headache （CEH）.Methods Totally 60 CEH patients from June 2012 to April 2013 were randomly divided into observation group （30 cases） receiving gabapentin combined with nerve block and control group （30 cases） given nerve block.Visual analog scales （VAS） and quality of sleep （QS） were used to evaluate the curative effects 1 week,1 and 3 months after treatment.The adverse reactions were observed.Results One week,1 and 3 months after treatment,the VAS score and QS score were significantly increased compared with those beforeer treatment in observation group [（1.5 ± 0.6）,（0.6 ± 0.5）,（1.8 ± 0.7） scores vs （6.8 ± 2.5） scores;（1.9 ± 0.2）,（1.1 ± 0.1）,（0.9 ± 0.2） scores vs （4.5 ± 0.4） scores] and control group [1.7 ±1.2）,（1.1 ±0.6）,（3.3 ± 1.3） scores vs （6.7 ±2.8） scores,（3.6 ±0.3）,（2.7 ±0.4）,（2.5 ±0.6） scores vs （4.6 ±0.3） scores],and they were significantly lower in observation group compared with those in control group after treatment （P 〈 0.05）.Three months after treatment,the cure rate and overall response rate in observation group were significantly higher than those in control group [73.3% （22/30） vs 53.3% （16/30）,100.0% （30/30） vs 90.0% （27/30）] （P 〈 0.05）.No serious adverse reactions occurred in both groups.Conclusion Gabapentin combined with nerve block is effective and safe in treating CEH,and it is superior to single nerve block.
