摘  要:目的探讨心脏彩超在诊断慢性心力衰竭中的临床价值,为临床诊断、判断临床疗效提供经验。方法回顾性分析2012年1月~2014年12月充血性慢性心力衰竭患者69例的临床资料,所有患者均采用心脏彩超测量患者左心室射血分数、左心室舒张末期内径、左心房内径,根据射血分数分为LVEF异常组和LVEF正常组,分析心脏彩超在慢性心力衰竭诊断以及评估临床疗效中的价值。结果慢性心力衰竭患者平均LVDd(52.4±11.6)mm接近正常值的上限,LAD(47.6±9.9)mm显著高于正常值,LVEF(37.9±11.3)%显著低于正常值。69例慢性心力衰竭患者,21例患者LVEF正常,平均(64.1±13.6)%,48例患者LVEF异常,平均(33.5±10.3)%。治疗前后,LVEF正常组患者LVEF变化不明显,而LVEF异常的患者治疗后LVEF显著提高。结论心脏彩超是诊断慢性心力衰竭的有效方法,具有无创的优点,还可以作为临床治疗效果的可靠检测指标。Objective To discuss clinical significance of echocardiography in the diagnosis of chronic heart failure, to provide experience for diagnosis and analysis of clinical efficacy. Methods Clinical data of 69 cases with congestive heart failure from Jan 2012 to Dec 2014 were respectively analyzed. All cases were detected about LVEF, LVDd, LAD by echocardiography, and were divided into group of abnormal LVEF and group of normal LVEF according to LVEF.Value of echocardiography in the diagnosis of chronic heart failure and evaluation of clinical efficacy were analyzed.Results Mean LVDd of chronic heart failure was(52.4±11.6)mm,which was near to upper limit of normal; LAD was(47.6±9.9)mm, which was higher than normal level; LVEF was(37.9±11.3)%, which was lower than normal level. Among69 cases with chronic heart failure, LVEF of 21 cases was normal, mean(64.1±13.6)%,and 48 cases were abnormal,mean(33.5±10.3)%. After treatment, LVEF of group of normal LVEF changed less; LVEF of group of abnormal LVEF after treatment increased significantly compared with before treatment. Conclusion Echocardiography is an effec tive method for diagnosis of chronic heart failure, with the advantage of non-invasive, and can be used as a reliable index for clinical therapeutic effect.
