认知干预护理对胃癌手术患者疾病不确定感的影响

摘　　要：目的研究认知干预护理对胃癌术后住院患者疾病不确定感的影响。方法选择胃癌手术患者90例为研究对象,分为干预组和对照组各45例。干预组采用认知干预护理,对照组采用常规护理,评估两组患者的心理及生理指标,分析护理干预对患者不确定感的影响。结果干预组干预后心率、收缩压、舒张压较干预前显著下降(P<0.01);干预后干预组心率、收缩压、舒张压也显著低于对照组(P<0.01);两组干预前后呼吸、淋巴细胞计数比较差异无统计学意义(P>0.05)。干预前两组患者疾病不确定感得分比较差异无统计学意义(P>0.05);干预后,干预组不确定性因素、复杂性因素以及总分得分均显著低于干预前(P<0.01);并且干预后,干预组不确定性因素、复杂性因素以及总分得分均显著低于对照组(P<0.01)。结论对患者进行护理干预,能够显著缓解患者的疾病不确定感,从而改善患者的预后。Objective To discuss the effect of cognitive intervention care on uncertainty in illness for patients with gastric carcinoma. Methods 90 cases with gastric carcinoma were selected and divided into intervention group and control group（each group of 45 cases）. The intervention group was nursed by cognitive intervention care, and control group was nursed by normal care. Psychological and physiological indicators were detected, and effect of nursing intervention on uncertainty in illness was analyzed. Results After intervention, heart rate, systolic blood pressure and diastolic blood pressure of intervention group were lower（P〈0.01）; Compared with control group, heart rate, systolic blood pressure and diastolic blood pressure of intervention group after intervention were lower（P〈0.01）. Breathing, lymphocyte count of two groups after intervention showed no significant difference（P〉0.05）; After intervention, uncertainties dimension,complexity factors dimension and total scores of intervention group were lower intervention; Compared with control group, uncertainties dimension, complexity factors dimension and total scores of intervention group after intervention were lower（P〈0.01）. Conclusion Uncertainty in illness is lowered, and prognosis is improved with nursing intervention for patients with gastric carcinoma.
