益母草注射液联合卡孕栓预防双胎妊娠剖宫产产后出血的观察

摘　　要：目的观察益母草注射液联合卡孕栓预防双胎妊娠剖宫产术中、术后出血的疗效。方法选择2013年1月~2014年12月我院双胎妊娠剖宫产分娩的产妇180例,分为研究组和对照组。研究组：剖宫产术中切开子宫肌层时舌下含服卡孕栓1 mg,胎儿娩出后子宫肌壁注射益母草2 m L、缩宫素20 IU,术后每隔12 h肌肉注射益母草2 m L,共3 d。对照组：胎儿娩出后子宫肌壁注射缩宫素20 IU,同时静脉点滴缩宫素20 IU,术后缩宫素20 IU肌肉注射,每日2次,共3 d。观察比较两组术中、术后2 h、术后2~24 h及24 h总出血量、产后出血发生率。结果研究组术中、术后2 h、术后2~24 h及24 h总出血量均少于对照组（P〈0.05）;研究组产后出血发生率低于对照组（P〈0.05）。结论益母草注射液联合卡孕栓能有效预防双胎妊娠剖宫产产后出血的发生,减少术中、术后出血量。Objective To explore the effect of leonurus injection combined with carboprost in preventing intraoperatve and postpartium hemorrhage of cesarean section in twin pregnancy. Methods A total of 180 cases of twin pregnancy undergoed cesarean section from January 2013 to December 2014 in our hospital were randomly divided into research group and the control group. Research group was given sublingual containing carboprost 1 mg with cesarean section incision in the myometrium, after fetal childbirth uterine muscle wall injection 1eonurus 2 m L, shrinkage of oxytocin20 IU, postoperative every 12 h intramuscular injection of motherwort 2 m L, a total of three days. In the control group after delivery of fetus uterine muscle wall injection shrinkage oxytocin 20 IU, and intravenous infusion of oxytocin 20 IU,shrinking after operation, the oxytocin 20 IU intramuscular injection, two times a day, a total of three days. The total bleeding volume 2 h, 2 to 24 h, and 24 hours after operation and the incidence of postpartum hemorrhage were observed and compared between the two groups. Results The bleeding during operation, postoperative 2 h, after 2 to 24 h,and 24 hours total blood loss in research group were significantly less than the control group（P〈0.05）; The incidence of postpartum hemorrhage was significantly lower than the control group（ P 0.05）. Conclusion Leonurus injection and carboprost can effectively prevent the occurrence of postpartum hemorrhage of cesarean section in twin pregnancy and reduce intraoperative and postoperative blood loss.
