肿瘤专科医院铜绿假单胞菌分布及耐药性分析

摘　　要：目的了解肿瘤患者分离铜绿假单胞菌（PAE）的分布及耐药情况,为临床合理使用抗菌药物预防和控制感染提供依据。方法收集我院肿瘤专科2013年1月1日~2014年12月31日各类临床标本PAE分离株285株,检测PAE对常用的11种抗菌药物的敏感性,应用双纸片协同试验检测产金属β内酰胺酶情况。结果 285株PAE分离自血液17.89%（51/285）,痰、支气管灌洗液36.14%（103/285）,尿液5.97%（17/285）,胸腹水、脑脊液20.00%（57/285）,脓液、分泌物20.00%（57/285）。PAE对亚胺培南、氨曲南、哌拉西林的耐药率较高,依次为30.18%、21.75%、19.65%;对头孢他啶、哌拉西林/他唑巴坦、头孢吡肟、庆大霉素、环丙沙星、左旋氧氟沙星、妥布霉素、阿米卡星敏感率均〉70%。结论 PAE对亚胺培南、氨曲南耐药较高,而对氨基糖苷类、喹诺酮类、头孢吡肟、头孢他啶、哌拉西林/他唑巴坦的敏感率较高,可作为优先选择药物。Objective To investigate the distribution and drug resistance of the pseudomonas aeruginosa（PAE） isolated from tumor patients and provide reference for the clinical rational use of antibacterial agents to prevent and control infection. Methods A total of 285 strains of PAE isolated from various clinical specimens of our hospital from January 1,2013 to December 31, 2014 were collected. The sensitivity of PAE to 11 types of antibacterial agents was detected and the double-disk synergy test was used to detect the situation of metallo-β-lactamase. Results Of the 285 strains of PAE, 17.89%（51/285） was isolated from blood, 36.14%（103/285） from sputum and bronchial lavage fluid, 5.97%（17/285）from urine, 20.00%（57/285） from pleural effusion and cerebrospinal fluid, and 20.00%（57/285） from pus and serection.The drug resistance rates of PAE to imipenem, aztreonam and piperacillin were high, which were 30.18%, 21.75% and19.65% respectively. The sensitive rates of PAE to ceftazidime, piperacillin/tazobactam, cefepime, gentamicin vancomycin, ciprofloxacin, levofloxacin, tobramycin and amikacin were all larger than 70%. Conclusion The drug resistance rates of PAE to imipenem and aztreonam are high while the sensitive rates of PAE to aminoglycosides,quinolones, cefepime, ceftazidime and piperacillin/tazobactam are high, which can serve as the preferred drugs.
