无偿献血者梅毒ELISA不合格结果的TPPA确证分析

摘　　要：目的无偿献血者梅毒不合格结果的螺旋体颗粒凝集试验(Treponema pallidum particle agglutination assay,TPPA)确证分析,探讨灰区限设置的必要性和合理性。方法收集河北省血液中心498份梅毒螺旋体不合格标本,采用TPPA法进行确证试验。结果酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测不合格血液标本498份经TPPA法确证281份阳性,217份阴性,ELISA法假阳性率为43.57%;初次双试剂反应性标本:S/CO均>5,其阳性符合率98.32%;在确证为阳性的标本中,有265份呈ELISA双试剂反应性,有14份呈ELISA单试剂反应性,有4份ELISA检测结果处于灰区值。结论梅毒ELISA筛查实验存在部分假阳性的问题,为减少血源流失,可考虑对不合格标本进行确证试验,同时有必要设置合理的灰区范围。Objective To confirm and analyze the unqualifi donor, and to investigate the necessity and rationality to ed results for syphilis by ELISA from volunteer blood establish the criteria of grey zone. Methods 498 un- qualified blood samples for Treponema pallidumare detection were collected by Hebei province blood center. TPPA methods were employed as the confirmatory test. Results In these 498 samples with unqualified results by ELISA with confirmatory test by TPPA, found 281 samples are positive and 217 samples are negative for Treponema pallidumare detection. The false positive rate is 43.57%. First time double reagent reaction sam- pies, S/CO were both more than 5, with positive rate 98.32%. Among the TPPA positive 281 samples, there were 265 samples both reactive for two different ELISA reagents. While 14 samples were positive for only one ELISA reagent, and the other 4 were in the grey zone. Conclusions The false positive of syphilis can be ob- tained in the samples screened by ELISA. The results were confirmed further by TPPA in order to prohibit the loss of blood donor, it is also reasonable to establish the proper grey zone to ensure the blood safety.
