干扰素皮损内注射联合重组人干扰素凝胶外搽治疗尖锐湿疣的临床研究

摘　　要：目的:探究干扰素皮损内注射联合重组人干扰素凝胶外搽治疗尖锐湿疣的临床效果,为尖锐湿疣患者提供有效的临床治疗方案。方法:选取2013年4月至2014年4月来院就诊的120例尖锐湿疣患者为研究对象,随机分为对照组和观察组,各60例。对照组患者仅给予CO2激光治疗,观察组在对照组的基础上给予干扰素皮损内注射联合重组人干扰素凝胶外搽治疗,统计尖锐湿疣分布情况,并对比治疗前后两组患者细胞免疫功能、临床疗效及复发情况。结果:本研究统计发现男性疣体多发于包皮、阴茎及尿道口等部位,女性疣体多发于会阴、大小阴唇、阴道口等部位;两组患者经治疗后,细胞免疫功能均有所改善,与对照组相比,治疗后观察组CD8+、SIL-2R水平明显减小,CD4+、CD4+/CD8+、IL-2水平明显增加,且差异具有统计学意义(P<0.05),但无论对照组还是观察组,治疗前后CD3+差异无统计学意义(P>0.05);观察组、对照组治疗总有效率分别为95.0%、85.0%,6个月内总复发率分别为16.7%、40.0%,差异均具有统计学意义(P<0.05)。结论:干扰素皮损内注射联合重组人干扰素凝胶外搽治疗尖锐湿疣相比单纯的CO2激光治疗临床效果更加优良,且皮损内注射干扰素及重组人干扰素凝胶外搽操作方法简单,易于患者接受,值得临床推广使用。Objectives：To explore the clinical effects of intralesional interferon injections combined with re-combinant human interferon gel for treatment of genital warts,providing effective clinical treatment suggestion for pa-tients with genital warts.Methods：The 120 genital warts patients in our hospital from April 2013 to April 2014 were randomly divided into control group and observation group,60 cases in each.The control group was given only CO2 laser treatment,while the control group was given additional intralesional interferon injection combined with re-combinant human interferon gel.The statistical distribution of genital warts and the immune function before and after treatment,clinical efficacy and recurrence groups of the two groups were compared.Results：The study found that male wart was mainly in the foreskin,penis and urethra and other parts,while female wart was mainly in the perine-um,labia,orificium vaginae and other parts;after treatment,the cells immune function was improved in both groups;compared with the control group,the CD8 ＋,SIL -2R levels in observation group were significantly re-duced after treatment,while the CD4 ＋,CD4 ＋ /CD8 ＋,IL -2 levels were significantly increased,with statisti-cally significant difference （P 〈0.05）,but difference in CD3 ＋ level in both groups was not significant （P 〉0.05）;the total efficiency of observation group and control group was 95.0% and 85.0% respectively,and the to-tal recurrence rate within 6 months was 16.7%,40.0% respectively,both with statistically significant difference （P 〈0.05）.Conclusions：Intralesional interferon injection combined with recombinant human interferon gel is of better clinical effects than simple CO2 laser treatment with easy access,which is worthy of clinical application.
