摘 要:目的探讨雷公藤多苷联合贝那普利及肝素对小儿过敏性紫癜性肾炎24h尿蛋白及尿红细胞水平的影响。方法选择我院2013年2月~2014年12月收治83例过敏性紫癜性肾炎患儿展开研究,随机分为两组。对照组应用贝那普利与肝素钠治疗,观察组在对照组基础上加用雷公藤多苷片,比较两组治疗前后24h尿蛋白与尿红细胞水平、治疗总有效率与不良反应发生率。结果两组治疗前24h尿蛋白与尿红细胞的比较差异均无统计学意义(P〉0.05);经治疗后两组均获得改善,组内前后对比差异有统计学意义(P〈0.05),且治疗后观察组24h尿蛋白与尿红细胞均低于对照组,差异有统计学意义(P〈0.05)。观察组与对照组不良反应发生率分别为7.1%、4.9%,差异无统计学意义(P〉0.05);观察组治疗总有效率为95.2%,高于对照组80.5%,差异有统计学意义(P〈0.05)。结论雷公藤多苷联合贝那普利及肝素治疗小儿过敏性紫癜性肾炎可有效降低患儿24h尿蛋白及尿红细胞水平,血尿与蛋白尿症状得到改善,修复肾功能,且具有较高安全性,可作为理想治疗方案。Objective To investigate and analyze the effect of tripterygium glycosides combined with benazepril and heparin on 24-hour urine protein and red blood cell levels in children with henoch-schonlein purpura nephritis. Methods The study was conducted on 83 children with henoch-schonlein purpura nephritis admitted in our hospital from February 2013 to December 2014, and all the cases were randomly divided into two groups. The control group re- ceived benazepril and heparin treatment, and the observation group received tripterygium glycosides tablet based on the control group.24-hour urine protein and red blood cell levels in urine, total effective rate and incidence of adverse reactions of the two groups before and after treatment were compared. Results Before treatment, there were no statistical significances in 24-hour urine protein and red blood cell levels(P〉0.05). After treatment, 24-hour urine protein and red blood cell levels in the both groups improved, and differences of 24-hour urine protein and red blood cell levels in the two groups before and after treatment were statistically significant(P〈0.05). 24-hour urine protein and red blood cell levels in the observation group after treatment were lower than those in the control group, and the differences between the two groups were statistically significant(P〈0.05). Adverse reactions rates in the observation group and the control group were 7.1% and 4.9% respectively, and the difference was not statistically significant (P〉0.05). The total effec- tive rate of the observation group was 95.2%, higher than 80.5% of the control group, and the difference was statisti- cally significant(P〈0.05). Conclusion Tripterygium glycosides combined with benazepril and heparin in treating children with henoch-schonlein purpura nephritis can effectively reduce the 24-hour urine protein and red blood cell levels, improve hematuria and proteinuria symptoms', recover renal function, and has high security. So it can be an ideal treat- ment.
