摘 要:目的:探讨护士参与的联合饮食管理在妊娠糖尿病(GDM)患者管理中的应用效果。方法:选择住院GDM患者58例采用随机数字表分为干预组和对照组各29例,对照组由责任护士制定饮食方案,采用常规GDM糖尿病饮食教育;干预组在常规教育基础上构建以护士为主体的饮食管理组织结构,明确GDM饮食管理目标,制订及实施GDM患者营养风险筛查评估、个体化GDM饮食方案、个性化饮食教育、饮食管理制度等管理。比较两组患者入院时及干预3个月糖尿病饮食知识掌握情况、血糖控制情况、对住院期间糖尿病饮食管理的满意度。结果:干预3个月,两组糖尿病饮食运动知识评分均较干预前提高,且干预组显著高于对照组(P<0.01);干预组空腹血糖、三餐后2 h血糖、糖化血红蛋白值均低于对照组(P<0.01),对住院期间糖尿病饮食管理的满意度显著高于对照组(P<0.05)。结论:采用护士参与的联合饮食管理能提高护士、医生、营养师对GDM饮食管理能力,提高患者满意度,是目前较好GDM饮食管理模式。Objective: To investigate the effect of the combined dietary management with the participation of nurses in the management of patients with gestational diabetes mellitus( GDM). Methods: 58 patients with GDM were randomly divided into the intervention group and the control group( 29 cases in each group). The patient's dietary recipe was suggested by primary nurse and the patients were given routine health education in the control group; in the intervention group the dietary management objective was set up and the individualized dietary management was implemented by nurses only. The mastery of the diabetic diet knowledge,blood glucose control and satisfaction of the patients with dietary management during hospitalization were compared between the two groups on admission and after the intervention for 3 months. Results: After the intervention for 3 months,the scores of patient' s mastery of the diabetic diet and exercise knowledge increased in the two groups compared with those before the intervention and they were significantly higher in the intervention group than the control group( P〈0. 01); the level of fasting blood glucose and postprandial 2h blood glucose of three meals and the value of glycosylated hemoglobin in the intervention group was significantly lower than the control group( P〈0. 01); the satisfaction rate of the patients with the combined dietary management during hospitalization was significantly higher in the intervention group than the control group( P〈0. 05).Conclusion: Application of the combined dietary management with the participation of nurses can improve the capability of nurses,doctors and dietitian in the management of patients with GDM and can improve patient satisfaction.
