摘 要:目的研究颅脑外伤术后引起颅内感染的相关因素。方法回顾性分析1723例颅脑外伤患者的资料,其中术后并发颅内感染者139例,通过单因素及多因素分析有关的危险因素。结果单因素分析结果显示,颅内感染与脑室外引流、脑脊液漏、手术时间(≥4 h)、手术次数(≥2次)、低蛋白血症症(是)、开放性伤口、术前格拉斯哥昏迷评分(GCS)和年龄(≥60岁)相关(P<0.05)。多因素分析显示,发生颅内感染的独立危险因素是脑脊液漏、手术次数(≥2次)、手术时间和低蛋白血症相关(P<0.05)。结论颅脑外伤术后患者会因多种因素的共同作用而并发颅内感染。手术时间≥4 h、脑室外引流与脑脊液漏是发生颅内感染的独立高危因素,临床上应对此类患者尽早采取预防措施,避免发生术后颅内感染。ObjectiveTo research related factors of postoperative intracranial infections in craniocerebral trauma.MethodsClinical data of 1723 craniocerebral trauma patients were retrospectively analyzed. Among them, there were 139 cases with complicated intracranial infections. Univariate and multivariate analysis were applied to analyze related risk factors.ResultsUnivariate analysis showed that intracranial infections were related with external ventricular drainage, cerebrospinal fluid leakage, operation time (≥4 h), operation times(≥2), hypoproteinemia(with), open wound, preoperative Glasgow coma scale (GCS) score, and age (≥60 years old) (P〈0.05). Multivariate analysis showed that independent factors of intracranial infections included cerebrospinal fluid leakage, operation times(≥2); and operation time was correlated with hypoproteinemia(P〈0.05). ConclusionMultiple factors in craniocerebral trauma patients will lead to intracranial infections. Independent high-risk factors include operation time≥4 h, external ventricular drainage, and cerebrospinal fluid leakage. Early implement of prevention measures are essential for avoiding postoperative intracranial infections.
