子宫内膜异位症对体外受精-胚胎移植妊娠结局的影响

摘　　要：目的探讨子宫内膜异位症(EMs)对体外受精-胚胎移植(IVF-ET)妊娠结局的影响。方法 155个周期EMs患者(A组,EMs组)以及同期309周期输卵管因素患者(B组,对照组)行IVF-ET,比较年龄、不孕年限、降调时间、Gn使用天数、Gn总量、HCG日子宫内膜厚度、HCG日E2值、获卵数、2PN率、卵裂率、优胚率、移植胚胎数、种植率、临床妊娠率、流产率并分析。结果内异症组Gn使用天数、获卵数、移植胚胎数显著低于对照组(P<0.01),其他指标两组无差异(P>0.05)。EMs组临床妊娠率较对照组有下降趋势[(40.65%(63/155)vs 42.72%(132/309))(P>0.05)、流产率较对照组有上升趋势[17.46%(11/63)vs 15.15%(20/132)](P>0.05)。结论 EMs患者卵巢储备功能下降,获卵数和可移植胚胎数显著下降,Gn使用天数、妊娠率有下降趋势,流产率有增加趋势。Objective：To explore the effect of endometriosis on in vitro fertilization and embryo transfer（IVF-ET）according to the comparison between patients with endometriosis or tubal factor. Methods：155 patients with endometriosis（EMs group）and 309 tubal factor women（control group）underwent IVF-ET were analyzed including age,Infertility duration,down-regulation duration,Gn duration,Gn dosage Endometrial thickness and E2 level on h CG injection day,number of oocyte retrieved,mature oocyte rate,cleavage rate,Good-quality embryos rate,number of embryos transferred,implantation rate,clinical pregnancy rate and abortion rate. Results：Gn duration,number of oocyte and number of embryos transferred in EMs group were significantly lower than those in control group（P﹤0.01）. There were no significant difference in other data. Conclusion：EMs patients had a poor ovarian response,which were associated wit decreased number of oocytes and embryo transferred,a lower clinical pregnancy rate and a higher abortion rate.
