神经阻滞治疗带状疱疹疼痛临床观察

摘　　要：目的：探讨神经阻滞治疗带状疱疹疼痛的临床疗效及对患者血清Th细胞因子的影响。方法：将我院2012年6月~2014年8月收治的72例带状疱疹疼痛患者作为研究对象,采用随机数字表法分为对照组和观察组。对照组采用药物治疗,观察组在对照组的基础上加神经阻滞法治疗。于治疗后进行视觉模拟评分（VAS）、汉密顿焦虑量表（HAMA）评分和疗效评价,经酶联免疫吸附法测定细胞因子。结果：与对照组比较,观察组治疗后2周、3周和5周的VAS评分明显降低（P〈0.05）;治疗后5周,观察组总有效率（89.89%）和显效率（63.89%）均明显高于对照组（66.67%,38.89%）,HAMA量表总分低于对照组（P〈0.05）。患者血清中的TNF-α、IL-2、IL-4细胞因子治疗前后无明显差异（P〉0.05）。结论：带状疱疹疼痛患者机体细胞免疫功能减退,神经节内潜伏的病毒易被激活,神经阻滞联合药物治疗能迅速缓解疼痛并有预防作用,但对机体免疫调节功能无明显作用。Objective： To investigate the effect of nerve blocking in the treatment of herpetic zoster pain and its influence on the serum levels of Th cytokines.Methods：From Jun 2012 to Aug 2014,72 patients with herpes zostcr pain in our hospital were randomly divided into control group and observation group.The control group was treated with medicine, while the observation group was treated with nerve block on the basis of the control group. In after the treatment by visual analogue scale （VAS）,ftamilton Anxiety Scale （HAMA） score and efficacy evaluation,by enzyme linked immunosorbent assay determination of cytokine.Results：Compared with the control group,the P〈0.05 score of the observation group was significantly lower （VAS） after 2 weeks, 3 weeks and 5 weeks alter treatment, the total effective rate （89.89%） and 5 （63.89%） were significantly higher than those in the control group （38.89%, 66.67%）, and the total score of llAMA was lower than that of the control group （P〈0.05）.There was no significant difference in serum TNF-α, IL-2, and IL-4 cells （P〉0.05）.Conclusion：The cellular immune function in patients with herpes zoster pain is decreased, the latent virus in the ganglion is activated, and the nerve block combined with drug therapy can quickly relieve pain and prevent it.
