逆行髓内钉与锁定钢板治疗股骨远端骨折的随访观察

摘　　要：目的:探究对股骨远端骨折患者行锁定钢板和逆行髓内钉治疗的临床效果。方法:选取2008年1月至2010年2月我院股骨远端骨折患者68例,按数字顺序分A、B组各34例,分别行逆行髓内钉治疗、锁定钢板治疗,对比两组疗效。结果:A组行逆行髓内钉治疗后临床优良率较B组无统计学意义(P>0.05),但手术时间较B组长,术中出血量较B组大,术中透视次数、负重功能锻炼时间、骨折愈合时间较B组优,并发症发生率较B组高,差异有统计学意义(P<0.05)。结论:在股骨远端骨折治疗中,锁定钢板和逆行髓内钉均具有一定的疗效,应用依骨折类型合理选择术式,有利于调节患者关节功能、减少并发症。Objective： To explore the clinical effects of locking plates and retrograde intramedullary nail in the treatment of distal femoral fracture.Methods：68 cases of patients with distal femoral fractures admitted into the hospital during Jan 2008 to Feb 2010 were selected as the research objects.According to the sequence of nunlber, the objects were divided into group A and B with 34 cases in each group.The retrograde intramedullary nail and locking plates were performed respectively in the two groups.The efficacy of the two groups was compared.Results：After the given retrograde intramedullary nail treatment , the clinical excellent and good rate of group A was not statistically significant, compared with that of group B （P〉0.05）, but the operative time was longer than that in group B and the intraoperative bleeding volume was larger than that in group B.The times of intraoperative fluoroscopy, exercise time of weight-bearing function and fracture healing time were better than those in group B.The incidence rate of complications was higher than that in group B.The differences were statistically significant （P〈0.05）.Conclusion：In the treatment of distal femoral fi＇acturcs, locking plates and retrograde intramedullary nail were of certain effects.To select the operative mode reasonably according to the type of fractures is good for adjusting the joint lhnction of the patients and reducing complications.
