Prevention of Bone Cement Leakage in Osteoporotic Vertebral Compression Fractures Treated by Percutaneous Vertebral Angioplasty

摘  要:目的:探讨经皮椎体成形术治疗骨质疏松性椎体压缩性骨折并发骨水泥渗漏的预防方法。方法:应用经皮椎体成形术经双侧椎弓根入路治疗我院2011年2月至2014年12月收治的骨质疏松性椎体压缩性骨折119例130椎。结果:全部椎体均穿刺成功。治疗优良率98.3%。发生骨水泥渗漏12椎,漏入椎旁软组织4椎,漏入相邻椎间隙3椎,漏入椎体前缘5椎,骨水泥渗漏率9.2%,均无临床症状。亦无血管神经损伤、骨水泥椎管内渗漏等严重并发症的发生。结论:经皮椎体成形术虽有并发骨水泥渗漏的风险,但只要病例选择、椎弓根穿刺技术和骨水泥注射技术运用得当,渗漏是可以减少和避免的。Objective: To explore the prevention strategies of cement leakage in treatment of osteoporosis vertebral compression fracture(OVCF) by percutaneous vertebroplasty(PVP).Methods:From Feb 2011 to Dec 2014 in our hospital, 119 patients( 130 vertebrae)with OVCF were treated by PVP in bilateral pedicle approach.Results:All the vertebrae were punctured successfully.The excellent and good rate was 98.3%.There are 12 vertebrae in Bone cement leakage,leakage into the paravertebral soft tissue in 4 vertebrae,leakage into the adjacent intervertebral space in 3 vertebrae,leakage into the vertebral anterior edge in 5 vertebrae.The rate of bone cement leakage is 9.2%,and there were no clinical symptoms in all patients.There are no serious complications occurred,such as nervi vascularis injury or bone cement intraspinal leakage.Conclusion:PVP, although the risk of bone cement leakage complicated,but as long as choosing cases,pedicle puncture technique and bone cement injection technology used properly, the leakage is can be reduced or avoided.
