摘 要:目的 常规拍摄腰椎斜位片观察峡部,但制约因素较多,如肠气、肠内容物较多,常不能满足临床工作的需要。通过与普通腰椎斜位片比较,探讨数字断层融合技术(DTS)在腰椎斜位摄片中的应用价值。方法 选取60例腰椎疼痛患者,其中男性34例,女性26例;年龄19 ~ 72岁,平均年龄42.5岁。行普通斜位X射线平片和DTS成像,将2种图像的清晰度、对比度进行比较,分析其相应的表现和诊断。结果 60例腰椎疼痛患者所拍普通腰椎斜位片与DTS图像的峡部裂比率分别为25.0 %、38.3 %,可疑峡部裂比率分别为25.0 %、3.4 %,普通腰椎斜位片有50.0 %未见明显峡部裂,而DTS为58.3 %,由此可见,DTS图像质量高于普通成像。结论 DTS断层融合成像拍摄腰椎斜位图像清晰度高,避免了临近结构的重叠及肠气、肠内容物等其他因素的限制,辐射剂量低,价格低廉,在实际明确诊断腰椎峡部裂中起到很重要的作用。Objective To explore the application value of digital tomosynthesis(DTS) in lumbar spine oblique image by comparing with ordinary lumbar oblique image, and meet the clinical needs. Methods Sixty patients with lumbar pain were enrolled, which included 34 males and 26 females, aged 19- 72 years old with mean age of 42.5 years old. The ordinary oblique X-ray tomography fusion imaging and DTS were performed, and the clarity and contrast of images between 2 approaches were compared to analyze the corresponding performance and diagnostics. Results The rate of lumbar pain in 60 patients was 25.0 by ordinary lumbar oblique images and 38.3 % by DTS image, and suspicious spondylolysis rate was 25.0 % and 3.4 %, respectively. The 50.0 % were no significant spondylolysis in ordinary oblique images, and 58.3 % were no significant spondylolysis by DTS. The image quality of DTS was better than that of ordinary tomographic image. Conclusion It is demonstrated that DTS of lumbar oblique with high-definition, low-radiation dosage and low price, which avoiding limitations of intestinal gas, intestinal contents and adjacent structures, so it could play an important role in actual diagnosis of lumbar spondylolysis.
