超声检查在边界模糊的肾脏肿块中的诊断价值

摘　　要：目的 分析边界模糊的肾脏肿块的超声声像图特征及诊断价值。方法 选择104例边界模糊肾脏肿块住院患者，其中男性62例，女性42例；年龄12 ~ 85岁，平均年龄47岁。对比病理组织结果，对其超声图像特征进行总结，回顾性分析边界模糊的肾脏肿块超声声像图特征。结果 104例均经手术病理组织或针穿刺病理组织证实，恶性肿块81例，良性肿块23例。肾脏恶性肿块形态多不规则，内部回声多呈现低回声或强弱不等混合回声，且回声不均匀，部分患者存在液化坏死灶，肿瘤内血流形态呈点线状、网状或不规则的多样化表现，瘤内血流丰富，多以Ⅱ、Ⅲ级血流为主。肾脏良性肿块回声多呈混合回声，部分呈囊性回声，肿块内多无血流，少数以周边型血流为主，血流形态单一，呈点状或短线状，其内血液供应不丰富，以Ⅰ级血流为主。结论 超声检查对边界模糊的肾脏肿块良恶性鉴别诊断具有一定价值。Objective To analyze the ultrasonographic characteristics and diagnostic value in renal mass of fuzzy boundaries.Methods A total of 104 hospitalized patients with renal tumor of fuzzy boundaries were enrolled, which included 62 males and42 females, aged 12- 85 years old with mean age of 47 years old. Compared the pathological results, the ultrasonic image characteristics were summarized, and the ultrasonographic characteristics in renal mass of fuzzy boundaries were analyzed.Results All the 104 cases were confirmed by surgery pathology or needle biopsy, included 81 cases of malignant mass and 23 cases of benign mass. Most of kidney malignant mass showed irregular shape, the internal echo presented low echo or hybrid echo and uneven, the liquefied necrotic foci were existed in some patients. The morphologies of internal tumor showed linear, mesh or irregular, and blood supply was rich and largely of grade Ⅱ and grade Ⅲ. The echo of kidney benign mass mostly showed mixed echo and partial of cystic, and no blood flow in tumor, and the minority with peripheral blood flow. The morphology of blood flow was single,which showed dotted or short line, the blood supply was poor and largely of grade Ⅰ. Conclusion It is demonstrated that ultrasonic examination is valuable in differential diagnosis of renal benign and malignant mass of fuzzy boundaries.
