血管性血友病因子抗原、D-二聚体在多发性骨髓瘤中的意义

摘　　要：目的探讨血管性血友病因子抗原(v WF:Ag)、D-二聚体对多发性骨髓瘤(MM)诊断治疗及预后的临床意义。方法收治的68例多发性骨髓瘤患者为MM组,另同期体检健康者199例为对照组。两组v WF:Ag、D-二聚体的检测采用法国思塔高(STAGO)公司的STAR Evolution全自动血凝分析仪,试剂为法国思塔高公司的血凝配套试剂,严格按说明书操作。结果采用SPSS 18统计学软件处理,MM组和对照组间的v WF:Ag、D-二聚体比较采用F检验和t检验,MM治疗前和缓解后比较采用t检验。结果 v WF:Ag、D-二聚体水平在多发性骨髓瘤组明显高于正常健康组,两组间水平差异具有统计学意义(P<0.05),另外,MM治疗缓解后组比MM治疗前组的血浆v WF:Ag、D-二聚体水平有明显降低,两组间差异具有统计学意义(P<0.05)。结论动态监测血浆v WF:Ag和D-二聚体水平对多发性骨髓瘤的诊疗及预后有重要的临床意义。Objective To investigate the clinical significance of detection of von Willebrand factor antigen（v WF： Ag）and D-dimer in diagnosis, treatment and prognosis of multiple myeloma（MM）. Methods 68 patients with multiple myelomarecuited in MM group, while 199 healthy subjects as control group. v WF： Ag and D-dimer were detected and analyzed withreagent from a complete set of coagulation of the STAGO company,using STAR evolution automatic blood coagulation analyzerproduced by France STAGO company in strict accordance with the manual operation. The results were analyzed by SPSS 18.0,and the differences of v WF： Ag and D-dimer between MM group and control group were compared using F test and t test. Theresults of MM group before treatment and after mitigation were compared using t test. Results The levels of v WF： Ag and D-dimer in patients with multiple myeloma were higher than normal control group,and the differences between two groupsshowed statistical significance（P〈 0.05）, while the levels of v WF： Ag and D-dimer in patients with multiple myeloma after thetreatment were lower than that before treatment,and the differences showed statistical significance（P〈0.05）. Conclusion Dynamically monitoring the levels of plasma v WF： Ag and D-dimer possesses important significance for diagnosis, treatmentand prognosis of multiple myeloma.
