针刺联合益气养阴汤干预鼻咽癌放疗不良反应随机平行对照研究

摘　　要：[目的]观察针刺联合益气养阴汤干预鼻咽癌放疗不良反应疗效。[方法]使用随机平行对照方法,将80例门诊及住院患者按就诊顺序号法简单随机分为两组。根治性放疗,根治剂量66-70Gy,每次2Gy,5次/周,加强自身皮肤保护,高蛋白、高维生素饮食,做好口腔清洁。对照组40例益气养阴汤（玄参18g,北沙参、鸡血藤各15g,麦冬、天冬各12g,山豆根10g,生黄芪30g,生地、鸡内金、银花、枸杞各15g,半夏5g,女贞子15g;舌苔黄腻加佩兰、藿香;听力减弱或耳鸣加石菖蒲、磁石;食欲不振加麦芽;恶心呕吐加竹茹;咽痛加牛蒡子）,1剂/d,水煎200m L,3次/d。治疗组40例针刺：取穴双足三里、双下关、双颊车,坐位,华佗牌不锈钢毫针,直径0.35mm,长度45mm,1次/d,单侧15min/次,5次/周;汤药治疗同对照组。连续治疗14d为1疗程。观测临床症状、黏膜放射性损伤、放射性皮肤损伤、咽干、张口受限、不良反应。连续治疗4疗程,判定疗效。[结果]不良反应治疗组低于对照组（P〈0.05）。[结论]针刺联合益气养阴汤干预鼻咽癌放疗不良反应疗效满意,无严重不良反应,值得推广。[Objective]Acupuncture combined with radiotherapy Yiqi Yangyin decoction adverse effect.[Method]randomized parallel-group method using the 80 cases of outpatient and inpatient treatment order number method is simple and randomly divided into two groups.Radical radiotherapy,radical dose of 66-70Gy,each 2Gy,5 times/week,to strengthen the skin＇s own protection,high protein,high vitamin diet,good oral hygiene.40 cases in the control group Yiqi Yangyin soup（Scrophulariaceae 18 g,littoralis,Millettla the 15 g,Radix,aspartame each 12 g,tonkinensis 10 g,Health Astragalus 30 g,habitat,gallus gallusdomesticus,honeysu ckle,wolfberry the 15 g,Pinellia 5g,Ligustrumlucidum 15g;Jia Peilan greasy yellow tongue coating,rugosa;hearing loss or tinnitus plus Shichangpu,magnets;loss of appetite Jiamai bud;nausea and vomiting plus Zhuru;sore throat plus Arctium）,1/d,decoction 200 m L,3 times/d.Acupuncture treatment group of 40 patients： acupoints feet three years,double Shimonoseki,cheeks car seat,Hua brand stainless steel needle,the diameter of 0.35 mm,length 45 mm,1 times/d,unilateral 15min/day,5 times/Zhou;decoction treatment with the control group.14 d for a course of continuous treatment.Observation of clinical symptoms,mucosal radiation injury,radiation-induced skin damage,throat,mouth limited adverse reactions.4 courses of continuous treatment,to determine efficacy.[Results]Adverse treatment group than the control group（P〈0.05）.[Conclusion]Acupuncture combined with radiotherapy Yiqiyangyin decoction adverse results were satisfactory,no side effects,it is worth promoting.
