摘 要:目的建立余姚地区正常孕妇妊娠各期的甲状腺激素参考值范围。方法以2013年6月-2014年6月在本市产检的孕妇为研究对象,采用化学发光仪对3 898例各孕期孕妇进行甲状腺激素检测,并统计分析得到各自的参考值范围。结果正常孕妇各孕期甲状腺激素水平以中位数(M)及双侧限值(P2.5~P97.5)表示其参考范围。TSH在早孕期[1.02(0.06~2.59)]m IU/L、中孕期[1.42(0.19~3.07)]m IU/L、晚孕期[1.51(0.41~2.98)]m IU/L逐渐上升。FT4在早孕期[15.11(11.79~18.61)]pmol/L、中孕期[13.50(10.61~17.12)]pmol/L、晚孕期[12.49(9.68~15.98)]pmol/L逐渐下降。与采用试剂盒的甲状腺成人(非妊娠)参考值相比,按照本研究建立的妊娠期特异性参考值进行诊断,亚临床甲状腺功能减退患病率增加了1.99%,差异有统计学意义(P〈0.01)。结论甲状腺激素在早孕期、中孕期、晚孕期间差异有统计学意义,建立适合本地区妊娠期妇女不同孕期的甲状腺激素参考值将有助于妊娠期甲状腺疾病的诊疗。Objective To establish the reference ranges of thyroid hormone of the normal pregnant women in Yuyao. Methods From June 2013 to June 2014, pregnant women in this city as the research object, the thyroid hormone levels in serum of 3 898 women in different gestation were detected by chemiluminescence instrument, and their reference range were established by statistical analysis. Results The reference ranges of thyroid hormone of the normal pregnant women in different gestation were calculated as median ( M value) and two - sided limits ( P2.5 - P97.5 ). The TSH was [ 1.02 ( 0.06 - 2.59 ) ] mIU/L for the early gestation, [ 1.42(0.19 -3.07) ] mIU/L for the middle gestation and [ 1.51 (0.41 -2.98 ) ] mIU/L for the late gestation, and gradually increased. The FT4 was was [ 15.11 ( 11.79 - 18.61 ) ] pmol/L for the early gestation, [ 13.50( 10.61 - 17.12) ] pmol/L for the middle gestation and [ 12.49 (9.68 - 15.98 ) ] pmol/L for the late gestation, and gradually decreased. Compared with adult thyroid reference values (not pregnant) which adopt reagent kits, by the pregnancy specificity reference values based on this research to diagnose, the the morbidity of subclinical thyroid hypofunction increaced by 1.99% , whose difference had statistical significance(P 〈 0.01 ). Conclusion The thyroid hormone levels showed significant deviation in the early, middle and late gestation. Therefore, establishing trimester - specific reference ranges of thyroid hormones in different gestation can help the diagnosis and treatment of thyroid diseases in pregnancy.
