摘  要:目的了解鲍曼不动杆菌(Acinetobacter baumanii,Ab)整合子分布情况,探讨Ⅰ类整合子与其耐药性的关系。方法分离2013年1月-9月本院临床Ab菌株,用VITEK 2全自动微生物鉴定系统进行菌种鉴定及药敏检测,PCR法检测Ⅰ类整合子及可变区。结果临床分离的Ab多数来源于痰液标本,科室以ICU/CCU居多。头孢哌酮/舒巴坦和左旋氧氟沙星耐药率较低,其余抗生素耐药率均>40%。Ⅰ类整合子检出率为19.0%;除头孢哌酮/舒巴坦、头孢曲松外,阳性菌株的耐药率均高于阴性菌株,其多重耐药率也明显高于后者,差异均有统计学意义(P<0.05)。整合子阳性菌扩增出两类可变区,分别含有aac A4-cat B8-aad A1及arr-3、aac A4、qac EΔ1、sul1基因盒。结论临床分离Ab的耐药率较高;I类整合子广泛存在Ab临床分离株,主要介导对氨基糖苷类、氯霉素类及磺胺类的耐药,并且与多重耐药性密切相关。Objective To investigate the distribution of class I integron of Acinetobacter baumanii(Ab) clinical isolates and to analyze the correlation between class I integron and drug resistance. Methods Acinetobacter baumanii were isolated from the clinic in our hospital from Jan. to Sep. 2013. VITEK 2 full automatic bio- identification system was used to detect the bacterial species and drug sensitivity, and class I integron and variable region were detected by PCR. Results The Ab strains were mainly from sputum samples and from ICU/CCU apartment. Except cefoperazone/sulbactam and levofloxacin, all the drug re- sistance rates of the other antibiotics were more than 40%. The detection rate of Class I integron was 19.0%, and the resist- ance rate of positive strains was higher than that of the negative strains except cefoperazone/sulbactam and ceftriaxone. And the muti - drug resistance was higher than the latter, and the difference had statistical significance ( P 〈 0.05 ). Two different stripes variable areas were applied by the positive strains of integron, which respectively contained aacΔ4 - catB8 - aadΔ1 and art - 3, aacΔ4, qacEΔ1, and sull. Conclusion Resistance rate of Ab in our hospital was high. Class I integron mainly exists in the clinical isolates of Ab, and are mainly resistant to aminoglycosides, chloramphenicol and sulfonamide, and has a close correlation with the multi -drug resistance.
