甘肃省农村学龄儿童碘营养状况及甲状腺功能分析

摘　　要：目的：了解甘肃省农村学龄儿童碘营养水平及甲状腺功能状态，为科学补碘提供理论依据。方法采集甘肃省武威市农村8～10岁学龄儿童血液、尿液样本，检测FT3、FT4、TT3、TT4、TSH、TgAb、TMAb及尿碘，同时测量其身高、体质量。结果男、女学生尿碘中位数分别为381．37μg／L和396．03μg／L ；TSH、FT3、FT4、TT3、TT4异常率分别为9．3％、2．1％、7．2％、5．2％、10．3％；TgAb与TMAb的阳性率均为4．1％；将尿碘水平分为3个组，在不同的尿碘水平上，TgAb、TMAb、TSH的差异均无统计学意义；甲状腺功能紊乱中以亚临床甲状腺功能减退症为主，占7.2％。结论学龄儿童碘营养过量，甲状腺功能紊乱以亚临床甲状腺功能减退症为主；应关注碘营养过量所带来的患甲状腺疾病的风险。Objective To explore the iodine nutritional status and thyroid function of school children in the countryside of Gansu Province ,and to provide a scientific basis for iodine supplementation .Methods Blood and urine samples of school children aged 8—10 years in Wuwei City were collected .Serum free three triiodothyronine （FT3 ） ,free thyroid hormone （FT4 ） ,triiodo thyronine （TT3 ） ,serum total thyroxine （TT4 ） ,thyroid＇stimulating hormone （TSH） ,thyroglobulin antibodies （TgAb） ,thyroid microsomai antibodies （TMAb） and urine iodine were detected .Children＇s height and weight were measured .Results The median urinary iodine of male and female students were 381 .37 μg/L and 396 .03 μg/L .The abnormal rate of TSH ,FT3 ,FT4 ,TT3 and TT4 were 9 .3% , 2.1% ,7 .2% ,5 .2% and 10 .3% .The positive rate of TgAb and TMAb were 4 .1% .When the urinary iodine level was divided into three groups , TgAb , TMAb , TSH were not statistically different . The mostly manifested was subclinical hypothyroidism ,accounting for 7 .2% of all thyroid function disorders .Conclusions Iodine level in school children is excessive .Subclinical hypothyroidism is the most common in all thyroid function disorders .We should be concerned about the risk of iodine overdose .
