摘　　要：目的了解患者Rh D抗原阴性患者在抢救时,输注与患者血型相同的Rh D阳性的血液。患者抗-D产生情况及效价水平。方法 Rh D抗原阴性患者输注Rh D抗原阳性血液后,5、10、20 d连续追踪3次,。使用微柱凝胶抗人球蛋白试剂卡检测患者红细胞不规则抗体及效价[1]。结果 15例Rh D抗原阴性的患者输注同型Rh D抗原阳性红细胞后,共有4例患者血清红细胞不规则抗体筛查阳性[2]。1例女性患者5 d血清红细胞不规则抗体筛查阳性,10 d后又有2例女性患者和1例男性患者血清红细胞不规则抗体筛查阳性。5 d后阳性标本抗体效价为4,20 d后抗体效价结果为16,3例10 d抗体效价为4,20 d后效价为16。结论 Rh D阴性患者输注同型Rh D阳性血液后,机体被所缺失的抗原免疫刺激,产生免疫性抗-D。抗体产生的几率与抗体效价的增高和输注次数及抗原量的多少呈正相关系数上升[3]。Objective To observe the patients＇anti-D antibody production and their titer level when rescuing the RhD negative patients by transfusing them with RhD positive blood. Methods After the transfusion of RhD positive blood to RhD negative patients, patients were traeked on the 5th, 10th and 20th day. The mierocolumn gel anti-globulin reagent cards made by Changchun Bioxun Biotech Co., Ltd were applied to test patients＂ irregular blood cells and antibody titer. Results Among 15 RhD negative patients who were transfused with RhD positive blood, a total of 4 patients＇irregular antibody in serum were screened positive. One case of female patient~ serum irregular antibody was screened positive after 5 days. Two females and one male were also detected positive after 10 days. After 5 days, the ratio of positive antibody titer was 4 and the ratio was 16 after 20 days. In three cases, the antibody titer was 4 and became 16 after 20 days. Condusion After transfusion of RhD positive blood to RhD negative patients, the immune system was alerted by RhD antigens, thus, anti-D antibody was produced. The probability of production of antibody shows positive correlation and rises accordingly with the increase in antibody tater, transfusion frequency and quantity of antigens.
