摘  要:本研究旨在通过研发国内首个临床输血领域的管理与评估系统,为输血医疗、科研和管理的发展提供一种先进的新工具。研究方法主要由计算机和医学统计相关方法技术构成,临床输血智能管理与评估系统应用B/S的三层结构,Flex实现界面前台表示,My SQL进行数据的存储,构建过程中使用J2EE技术对本系统进行升级;将数据挖掘方法引入输血数据分析,对不同格式的输血基础数据的规范化与集成以及从数据库中识别出输血有效数据集。研究设定主题词诊断检索方式,将临床医生给出的不规范诊断归类到《国际疾病分类第十次修订本(ICD-10)临床版》的标准诊断中,便于使用者检索。首次研究并建立中国人群的输血信息系统和技术标准,通过与患者在医院HIS、LIS、PACS等系统所有输血相关信息的实时数据交互,实现了管理者对医疗机构临床用血的动态监控管理。为设置评价医师临床用血过程合理性的参数标准,提高输血申请合格率,制定新的标准化输血效果评价体系,降低患者无效输血率以及输血管理领域的多中心合作奠定了基础。To develop the first domestic management and evaluation system in transfusion medicine and to provide a new tool for advanced transfusion medicine, scientific research and management. Research method was composed of computer tech-niques and medical statistics. The clinical transfusion intelligent management and evaluation system applied B/S three-layered structure, user interface foreground demonstration and data storage accomplished by Flex and MySQL techniques, respectively. J2EE technique was applied to system upgrade. Data mining was introduced to transfusion data analysis, which helped to standardize and integrate data of different formats for blood transfusion and to identify valid blood transfusion data sets from the database. For users to retrieve data easily, key words from the diagnosis were applied in the retrieval methods, in addition to the categorization of the nonstandard diagnoses given by the clinicians to the standard diagnosis according to International Classification of Diseases, ICD-10. The transfusion information system and technology standards were first studied and established for Chinese populations. The monitoring and management of clinical blood transfusion was accom- plished through real-time data communication on all the relevant information on transfusion patients in clinical HIS, LIS and PACS systems. This study sets up rational clinical parameters for transfusion that improved the qualified rate of blood transfusion applications, establishes a new standardized evaluation system for assessing transfusion impact, reduces the rate of invalid transfusions, and lavs the foundation for cooperation among varied organizations in transfusion management field.
