保元实脾饮治疗慢性心力衰竭临床疗效观察

摘　　要：目的:观察保元实脾饮治疗慢性心力衰竭的临床疗效。方法:采用前瞻性随机对照研究方法,选择60例慢性心衰的住院患者,随机分为治疗组(n=30)和对照组(n=30)。两组均给予内科常规治疗,治疗组在内科常规治疗基础上给予保元实脾饮(人参、黄芪、炙甘草、厚朴、白术、木瓜、草果仁、大腹子、附子、茯苓、干姜),水煎服,每日1剂,连续治疗1周。比较两组患者治疗前后NT-pro BNP水平、心功能及中医症候改善情况,并观察不良反应发生情况。结果:治疗前两组患者NT-pro BNP水平无统计学差异,治疗后NT-pro BNP水平均下降,和治疗前相比有统计学差异(P<0.01),且治疗组和对照组相比差异有统计学意义(P<0.05)。治疗组治疗后中医症候有效率96.7%,对照组有效率76.7%(P<0.05);治疗组心功能改善有效率93.3%,对照组有效率73.3%,两组比较有统计学差异(P<0.05)。两组均未见不良反应,治疗前后两组患者的血尿常规及肝肾功能未见异常。结论:保元实脾饮能显著改善慢性心衰患者的临床症状,改善心功能,无不良反应。Objective ：To evaluate the clinical efficacy and safety of Baoyuan Shipi decoction for chronic heart failure. Methods： In the prospective randomized controlled trial ,60 cases with chronic heart failure were ran- domly divided into an experimental group （ n = 30） and a control group （ n = 30）. The two groups were admin- istered by basic treatment, and the experimental group was given Baoyuan Shipi decoction （ginseng, astraga-lus, licorice root, Atractylodes, Magnolia, papaya, grass, nuts, semen Arecae, aconite, tuckahoe and dried ginger） ,being decocted in water for oral dose, daily one dose for one week. Compare NT -proBNP levels, cardiac function and the improving level before and after treatment, and observe the occurrence of adverse re- actions between two groups after thirty days. Results： In the experimental group after treatment the TCM syn- drome efficiency was 96.7% ,and the effective rate in the control group was 76.7% （P 〈0.05） ;in the exper- imental group the effective rate of improving heart function was 93.3 %, and the effective rate in the control group was 73.3% （P 〈0.05 ）. The level of NT -proBNP in two groups was decreased after treatment in two groups,which had a statistically significant difference compared with that before treatment （P 〈 0.01 ）. Com- pare between the experimental group and the control group and the difference was statistically significant （P 〈 0.05）. Two groups had no adverse reactions. Blood and urine routine and hepatic and renal functions were normal. Conclusion： Baoyuan Shipi decoction can significantly improve the clinical symptoms in patients with chronic heart failure,improving the cardiac function, with no side effects.
