摘  要:目的:通过瞬时弹性成像扫描仪(Fibro Scan)以及彩色多普勒超声显像仪观察解毒化瘀颗粒治疗慢性乙型肝炎肝纤维化的临床疗效。方法:将200例乙型肝炎肝纤维化患者,随机分为解毒化瘀颗粒组和大黄蛰虫丸组,比较两组入组前基线特征,疗程30天,观察治疗前后患者肝脏彩超脾脏厚度、肝门静脉宽度、门静脉血流情况变化以及Fibro Scan扫描的肝硬度值(Stiffne值)的影响。结果:经过治疗后,治疗组患者脾脏厚度、肝门静脉宽度,门静脉流速(PVV)、肝动脉搏动指数(HA-PI)和肝血管指数(LVI)明显较治疗前降低,治疗组Fibro Scan扫描结果治疗前后比较,FS值显著下降(P<0.01),而对照组无明显变化(P>0.05)。结论:解毒化瘀颗粒对改善慢性乙型肝炎肝纤维化脾脏厚度、肝门静脉宽度、肝血流动力学、肝硬度值以及阻止其进程有良好疗效。Objective: To observe clinical curative effect of Jieduhuayu granule on liver fibrosis of Chronic hepatitis B by Instantaneous elastic imaging scanner (FibroScan) and Color doppler ultrasound imaging device. Method: 200 patients with liver fibrosis of hepatitis B were randomly divided into Jieduhuayu granule group and Rhubarb Pill group, compare the baseline characteristics of two groups before entering the group, had treatment for 30 days, observed the spleen thickness, width of hepatic portal vein, hepatic portal vein hemodynamics by hepatic ultrasound and the liver hardness value (Stiffne value) by FibroScan scanning before and after treatment. Result: After treatment, spleen thickness, width of hepatic vein, portal vein flow (PVV), hepatic artery pulsation index (HA - PI) and hepatic vascular index (LVI) of treatment group were significantly lower than which before treatment, compared the FibroScan scan results of treatment group before and after treatment, the FS value decreased significantly (P 〈 0.01), while the control group had no significant change (P 〉 0.05). Conclusion: has good curative effect on improving liver fibrosis of Chronic hepatitis B with the spleen thickness, width of hepatic portal vein, liver hemodynamics, liver hardness value and stopping its process.
