摘  要:目的筛选出系统性红斑狼疮活动组、稳定组与正常对照组血清中的差异蛋白,从整体水平寻找SLE患者血清的异常蛋白,寻找SLE的分子标记物。方法选用弱阳离子交换蛋白芯片(CM10芯片),用Protein Chip Biomarker System II-C型蛋白质芯片阅读仪读取芯片信息,应用表面增强激光解析离子飞行时间质谱(SELDI-TOF—MS)技术,分别检测SLE活动组40例、稳定组40例及正常人群40例血清的蛋白质质谱图,对不同分组数据进行差异表达统计分析、层次聚类分析和主成分分析,筛选出差异蛋白,并用生物信息学鉴定。结果 1 SLE活动组与稳定组的血清中有53个有统计学差异的蛋白峰(P<0.05),活动组血清中的差异蛋白有21个上调表达,32个下调表达;2 SLE稳定组与正常对照组的血清中共存在22个有统计学差异的蛋白峰(P<0.05),稳定组血清中的差异蛋白有15个上调表达,7个下调表达;3 SEL活动组与正常对照组的血清中共存在44个有统计学差异的蛋白峰(P<0.05),活动组血清中的差异蛋白有17个上调表达,27个下调表达;4应用生物学信息鉴定出铁调素、神经分泌蛋白生长因子片段、β淀粉样肽前体,活动组较正常组/稳定组表达升高;中性粒细胞防御素3和中性粒细胞防御素1、血浆蛋白酶c1抑制剂片段、补体C3片段、α-1抗胰糜蛋白酶,活动组较正常组/稳定组表达都下降;交联-α-胰蛋白酶抑制物H4重链片段,稳定组较正常组表达升高。结论 1 SLE患者血清中蛋白质表达和正常人血清有差异;2铁调素与狼疮肾炎活动有关,有望成为SLE活动的生物学标签;3中性粒细胞防御素3和中性粒细胞防御素1具有趋化作用、调理吞噬作用、细胞毒性、参与补体激活;4血清C3的下降可以成为诊断SLE的辅助性指标。Objective The identification of differentially expressed proteins in the serum of patients with active systemic lupus erythematosus (SLE), quiescent SLE, and normal control. To define the total serum levels of these proteins so as to uncover novel biomarkers. Methods The protein mass spectra of sera of 40 patients with active SLE, 40 patients with quiescent SLE and 40 normal controls were examined using SELDI-TOF-MS and weak cation exchange protein chip (CM10chip). The data on the chip were read using the Protein Chip Biomarker System II-C protein chip reader. Hierarchical clustering and principal component analysis of data from the different groups were performed. Proteins thus selected were identified using a bioinformatics database. Results 53 protein peaks on mass spectrometry of sera were significant differences between active SLE and quiescent SLE (p〈0.05). 21 of the proteins were up-regulated while another 32 were down-regulated in the active SLE group. 22 proteins peaks on mass spectrometry of sera were significant differences between quiescent SLE and normal controls (p〈0.05). 15 of the proteins were up-regulated while another 7 of them were down-regulated in the quiescent SLE. 44 proteins peaks on mass spectrometry of sera were significant differences between active SLE group and normal control (P〈0.05), 17 of them were up-regulated while 27 of them were downregulated in the active SLE group. The data of bioinformaties database revealed the the expression of hepcidin, neurosecretory protein growth factor and amyloid beta a4 proteinwas was increased in the active SLE group compared to that of the quiescent SLE group and normal control group; the ex-pression of neutrophil defensin 1, neutrophil defensin 3, plasma protease cl inhibitor fragment and complement C3 fragment and alpha-1-antiehymotrypsin was decreased in the active SLE group compared to that of the quiescent SLE group and normal control group; the expression of inter-alpha-trypsin inhibitor heavy chain h4 fragment was
