疑似下生殖道感染孕妇细菌和真菌分布及其抗生素敏感性分析

摘　　要：目的了解孕妇下生殖道感染病原菌及对抗生素敏感性,为临床合理用药提供依据。方法对本院2011年1月-2014年6月产科住院和产科门诊送检的480份宫颈分泌物培养中分离出的143株病原菌进行分析,并进行鉴定和体外药敏实验。结果假丝酵母菌46株,肠杆菌科细菌33株,葡萄球菌属细菌24株,肠球菌23株,β溶血性链球菌7株,淋球菌10株。肠杆菌科细菌对头孢西丁、头孢哌酮/舒巴坦、美罗培南的敏感率较高;葡萄球菌属对青霉素、苯唑西林、大环内酯类耐药率较高,对万古霉素、利奈唑胺、替考拉宁均敏感;肠球菌属细菌对氨苄西林的敏感率为78.26%,对高浓度庆大霉素敏感率为60.87%;假丝酵母菌对氟康唑的敏感率为58.69%。结论孕妇下生殖道感染菌种类多,耐药性严重,临床应重视病原学检查,根据细菌鉴定及药敏试验结果合理选择药物治疗。Objective To investigate distribution characteristics and antibiotic susceptibility of pathogens in lower geni- tal tract infection of pregnant women, and to provide the reference for clinical treatment. Methods A total of 480 cases of pregnant women was admitted in Obstetric Inpatient and Outpatient Department from January 2011 to June 2014. The isolated 143 strains were identified and their susceptibility to antibiotics was tested. Results A total of 143 strains was isolated of which 46 strains were fungi, 33 strains were enterobacteriaceae, and 24 strains were staphylococcus, 23 strains were enterococcus,7 strains were 13 -streptococcus hemo- lyticus, 10 strains were gonococcus. Enterobacteriaceae had a high susceptibility to cefoxitin, cefoperazone/ sulbactam, and meropenem. Staphylococcus demonstrated a strong resistance to penicillin, oxacillin, and macrolides, while they were sensitive to vancomycin, linezolid and teicoplanin. The antibiotic sensitive rate of enterococcus to ampicillin was 78.26% and to high - dose gentamicin was 60.87%. The antibiotic sensi- tive rate of fungi to fluconazole was 58.69%. Conclusion There were many different kinds of pathogenic bacteria in lower genital tract infection of pregnant women. The drug resistant condition was very serious. We should pay more attention to etiological examination. It is suggested that antibiotics should be used under the guidance of antibiotic susceptibility testing.
