摘　　要：目的评价北京市一起幼儿园水痘突发公共卫生事件的社会经济负担,为开展水痘疫苗免疫策略的经济学评价提供参考。方法采用回顾性调查方法,从社会和个人的不同视角,调查疫情给患者和参与疫情控制的机构带来的直接经济负担和间接经济负担。结果疫情持续24 d,共累及10例水痘病例。直接医疗费用263.3（95%CI=199.3~273.3）元/例,直接非医疗费用924.0（95%CI=805.1~1 042.9）元/例;间接经济负担4 682.9（95%CI=4 095.5~5 270.3）元/例,约为直接经济负担的3.9倍;总社会经济负担为5 843.2（95%CI=5 209.0~6 477.5）元/例。参与疫情控制机构承担的总社会经济负担为392 533元,包括人力资本投入35 9526元,物资/设备费11 097元,交通费5 740元,其他费用16170元。每例病例给防控机构造成的社会经济负担为39 253元,是患者个人社会经济负担（5 843.2元）的6.7倍。结论水痘突发事件给疫情控制机构带来的社会经济负担远超过给个人带来的社会经济负担;未来开展水痘疫苗免疫策略经济学评价时,应全面考虑疾病的社会经济负担。Objective To evaluate the social economic burden of a varicella outbreak defined as public health emergency in a kindergarten in Beijing. Methods Retrospective survey method from both social and individual viewpoints was conducted to investigate the direct and indirect economic burden of patients and institutes participating in disease control. Results Totally 10 varicella eases were involved in the outbreak lasted 24 days： Direct medical costs was 263.3 yuan（95% CI= 199.3-273.3）/case; Direct non-medical costs was 924 yuan（ 95%C1：805.1-1 042.9）/case, 3.5 times of the direct medical costs; Indirect economic bur- den was 4 682.9 yuan（95%Cl= 4 095.5-5 270.3）/case,3.9 times of direct economic burden; Social economic burden was 5 843.2 yuan（95%Cl= 5 209.0-6 477.5）/case. The total social economic burden of institutes participating in disease control was 392 533 yuan,including human capital investments 359 526 yuan, material/equipments costs 11 097 yuan, transportation costs 5 740 yuan and other costs 16 170 yuan. The social economic burden of institutes participating in disease control was 39 253 yuan/case, 6.7 times of the social and economic burden of the individual causes. Conclusion Social economic burden of institutes participating in disease control is far more than that of cases in varicella outbreak defined as public health emergency. For the economic evaluation of varicella vaccine immunization strategy in the future. Social economic burden should be fully taken into account.
