复方尼群洛尔对轻中度高血压患者血脂的影响——单一治疗组的前瞻性研究

摘　　要：目的：观察复方尼群洛尔对轻中度高血压患者血脂水平的影响，为评估尼群洛尔的安全性提供依据。方法：对东海县和海安县10个村的18～70岁常住居民开展高血压筛查，共入选341名轻中度高血压患者，使用复方尼群洛尔片治疗6个月，监测患者的血压、不良反应、严重事件和血脂等。结果：治疗6个月后高血压患者的血压控制率为90.91％；高血压患者治疗后甘油三脂和高密度脂蛋白胆固醇水平显著上升（ P＜0.001），总胆固醇水平变化不大（P＞0.05）。男性和女性患者高密度脂蛋白水平显著上升，异常率显著下降；甘油三酯水平男性患者显著上升，女性患者虽有上升但差异无统计学意义；男性和女性患者治疗前后总胆固醇水平无显著变化。结论：复方尼群洛尔片能够显著影响高血压患者血脂水平，尤其是升高高密度脂蛋白胆固醇，其确切机制有待进一步研究。Objective： To observe the effects of nitrendipine/atenolol combination tablets（abbreviation as Nile） on the blood lipid level in patients with mild-to-moderate essential hypertension, and to provide evidences for the safety assessment of Nile.Methods：10 villages were selected from Donghai and Haian County, Jiangsu province, and the residents aged from 18-70 years old were cluster sampled for hypertension screening.341 patients with mild-to-moderate essential hypertension were recruited and treated with Nile for 6 months.The blood pressure, adverse effects, severe events, blood lipid and so on were monitored regularly.Results：The control rate of blood pressure in the patients was 90.91%.The levels of triglyceride（ TG） and high density lipoprotein cholesterol（ HDL-C） were significantly increased after 6 month of treatment （ P 〈0.001 ） , however, the level of total cholesterol had not difference（ P 〉0.05 ） .The levels of HDL-C were significant increased and its abnormal rate was significantly decreased in male or female patients;the level of triglyceride in male patients was significant increased, it was also increased in female patients, however, the difference was not significant.The levels of total cholesterol had not significant change in male or female patients before and after treatment.Conclusion： Nile has a significant effect on the blood lipid level in hypertension patients, especially on increasing the HDL-C level.The mechanism of Nile on blood lipid level needs to be further studied.
