腹腔镜胆囊切除术中Rouviere沟引导定位的临床应用价值

摘　　要：目的:研究Rouviere沟引导定位对腹腔镜胆囊切除术的应用价值。方法:选择2010年7月至2014年10月在我院进行胆囊切除术的患者300例,根据手术方式不同将患者分为研究组和对照组,每组各150例。研究组患者在Rouviere沟引导下行腹腔镜胆囊切除术,对照组采用传统定位手术。观察并比较两组手术时间、并发症及手术效果。结果:研究组患者Rouviere沟分型为开放型89例(59.33%)、融合型32例(21.33%)、缺失型29例(19.33%),分型的性别构成存在统计学意义(P<0.05);与对照组比较,研究组手术时间较短、并发症少和中转开腹率低,差异有统计学意义(P<0.05)。两组术后未出现任何病例胆管损伤和手术死亡,手术后恢复较好。结论:Rouviere沟引导定位行腹腔镜胆囊切除术的临床效果较好,值得临床进一步推广应用。Objective： To study the application value of Rouviere groove guide on the orientation of laparoscopic cholecystectomy. Methods： 300 patients with laparoscopic cholecystectomy surgery in our hospital from July 2010 to October 2014 were selected. According to the different surgery methods, the patients were divided into the study group and the control group with 150 cases in each group. The patients in the study groupthe were taken the Rouviere laparoscopic cholecystectomy, while the patients in the control group were treated with the traditional operation. Then the operation time, postoperative complications and the clinical effects of patients in the two groups were observed and compared. Results： In the study group, 89 cases （59.33%） were the open type, 32 cases （21.33%） were fusion type and 29 cases （19.33%） were deletion type, and the classification of gender composition was statistical significance different （P〈0.05）; Compared with the control group, the operation time, postoperative complications and the convertion to laparotomy were better than those of the control group with statistically significant differences （P〈0.05）. Two groups of postoperative death did not appear any cases of bile duct injury and surgery, good recovery after the operation. Conclusion： Rouviere groove guide positioning line better clinical effect of laparoscopic cholecystectomy, which is worthy of clinical application.
