摘　　要：目的:探讨CRP、Fib、NT-pro BNP在早期心衰患者中的诊断价值,并通过ROC曲线方法找到临床诊断界值。方法:对48例房颤并发早期心衰患者(A组),31例为心房颤动并发重度心衰(B组)和40健康对照组(C组)血清Fib、CRP和NT-pro BNP水平。利用ROC曲线,评价CRP、Fib、NT-pro BNP房颤并发早期心力衰竭诊断效能。结果:CRP、Fib和NT-pro BNP在B组中水平最高,C组水平最低,三组间差异具有统计学意义(P<0.05)。NT-pro BNP的对心房颤动并发早期心力衰竭患者诊断的灵敏度和特异度明显高于Fib和CRP。NT-pro BNP曲线下面积明显高于Fib和CRP,差异有统计学意义(P>0.05)。结论:Fib、CRP和NT-pro BNP在房颤并发可能或早期心衰患者血清中水平升高,联合诊断可以提高并发可能心衰的诊断敏感性,使得临床及早干预。Objective： To explore of the diagnostic value of CRP, Fib and NT-pro BNP patients with chronic atrial fibrillation complicated with early heart failure, and find the clinical cut-off point of them using ROC curve method. Methods： Selected 48 cases of atrial fibrillation complicated with early heart failure as group A, 31 cases of atrial fibrillation complicated with severe heart failure as group B and 40 healthy cases as group C. The serum levels of Fib, CRP and NT-pro BNP of the three groups were detected. The diagnostic value of CRP, Fib and NT-pro BNP were evaluated by using ROC curve. Results： The levels of CRP, Fib and NT-pro BNP were highest in group B, and lowest in group C. There was statistically significant difference between the three groups（P 〈0.05）. The sensitivity and specificity of NT-pro BNP were significantly higher than those of Fib and CRP for patients with atrial fibrillation and early heart failure. The NT-pro BNP area under curve was significantly higher than that of Fib and CRP, and its difference was statistically significant（P 〉0.05）. Conclusions： The levels of Fib, CRP and the NT-pro BNP increased in patients with atrial fibrillation complicated with early heart failure. Joint diagnosis can improve the diagnostic sensitivity, and help to make clinical early intervention
