肝移植受者一般自我效能对其治疗依从性的影响

摘　　要：目的调查肝移植术后患者治疗依从性和一般自我效能水平,并分析一般自我效能对治疗依从性的影响。方法采用便利抽样的方法,选取2011年10月至2014年5月在解放军第302医院肝移植中心进行随访的肝移植术后患者204例,调查患者的一般资料、一般自我效能感和术后治疗依从性情况,分析其一般自我效能与治疗依从性之间相关性,以及患者术后治疗依从性的影响因素。结果肝移植受者一般自我效能与总依从性及服药、自我监测护理、生活习惯和随访依从性之间均呈高度正相关(P<0.01),依从性总分和一般自我效能得分呈线性相关,Pearson相关系数为0.94。一般自我效能、术后是否出现并发症、术后时间对术后治疗依从性的影响,具有统计学意义(P<0.05),其中一般自我效能对治疗依从性的影响最大。结论肝移植术后患者一般自我效能是治疗依从性的影响因素,可以通过探索提高患者一般自我效能的方法,来提高肝移植患者术后治疗依从性。Objective To explore the self-efficacy level and compliance after liver transplantation and to analyze the influence of the self-efficacy on compliance of patients after liver transplantation. Methods From October 2011 to May 2014,a total of 204 cases underwent liver transplantation were investigated in the People＇s Liberation Army （PLA） 302 Hospital liver transplant center. The general information,self-efficacy and compliance after operation were collected,and the influence factors of the postoperative compliance as well as the correlation between the self-efficacy and compliance were analyzed. Results The compliance was influenced by self-efficacy,postoperative complications and the length of time after surgery,and the results showed a remarkable difference（P〈0.05）,and self-efficacy was the most important influence factor. General compliance,taking medication,self-monitoring of care, life style and follow-up were positively correlated with self-efficacy（P〈0.01）. There was a positive correlation between the score of compliance,and self-efficacy correlation coefficient was 0.94. Conclusions Self-efficacy is one of the most critical influencing factors on compliance of liver transplant recipients. There is a positive correlation between self-efficacy and compliance. A new approach to improve the self-efficacy is to be achieved to improve the compliance of patients after liver transplantation.
