摘  要:目的探讨组织因子(TF)测定对评估全膝关节置换术后下肢深静脉血栓(DVT)形成的意义。方法对167例全膝关节置换术患者,采用酶联免疫吸附试验(ELISA)法检测患者血浆中TF和TF途径抑制物(TFPI)水平,对其结果进行Spearman相关分析和受试者工作特征(ROC)曲线分析,利用反转录-聚合酶链反应(RT-PCR)检测术后6h(T1)、术后12h(T2)、术后24h(T3)和术后48h(T4)时TFmRNA表达。结果Spearman相关分析显示,DVT发生与患者血浆TF、TFPI和血小板(PLT)水平呈正相关;ROC曲线分析显示,血浆TF水平预测DVT发生曲线下面积为0.794,灵敏度为75.0%,特异度为75.9%,血浆TFPI水平检测分别为0.752、80.7%和62.0%;RT-PCR结果显示,术后6h后TFmRNA表达量逐渐增加,术后24h达最大量,术后48h开始下降。结论行全膝关节置换术患者血浆TF和TFPI水平与术后DVT发生密切相关。Objective To investigate the significance of tissue factor detecting for predicting deep vein thrombosis after total knee arthroplasty. Methods The plasma tissue factor (TF) and tissue factor pathway inhibitor (TFPI) levels in 167 patients undergoing total knee replacement were detected by enzyme -linked immunosorbent assay (ELISA) and analyzed by Spearman correlation and receiver operating characteristic (ROC) analysis. The expression of TF mRNA was detected by reverse transcription - polymerase chain re- action (RT - PCR). Results Spearman correlation analysis showed that the occurrence of deep vein thrombosis (DVT) was positively correlated with the levels of TF, TFPI and Platelet ( PLT, r = 0. 673, 0. 425 and 0. 481, P 〈 0. 05 for all). ROC curve analysis revealed that the area under the curve was 0. 794, sensitivity was 75.0%, and specificity was 75.9% by plasma TF levels predicting the occurrence of DVT, and those by plasma TFPI levels were 0. 752, 80. 7% and 62. 0%, respectively. Conclusion The levels of TF and TFPI in patients undergoing total knee arthroplasty were closely associated with post- operative DVT.
