摘 要:目的分析改良外侧胫骨平台骨折塌陷关节面复位方法的临床应用效果。方法选择2012年10月至2013年10月接受手术治疗的胫骨平台骨折患者作为研究对象,按照随机原则将所有患者分为接受常规治疗的对照组及接受改良塌陷关节面复位治疗的观察组。检测所有患者接受治疗前后的关节面塌陷高度、膝关节功能评分、膝关节活动度。结果1手术即刻及末次随访时,两组患者关节面塌陷高度均低于手术前(P<0.05),且观察组患者的手术后即刻及末次随访时关节面塌陷高度均低于对照组(P<0.05)。2治疗后16周,两组患者的膝关节功能评分均明显高于治疗后8周(P<0.05),且观察组患者的治疗后8周及16周的膝关节功能评分均显著高于对照组(P<0.05)。3治疗后16周,两组患者的膝关节最大活动度均明显大于治疗后8周(P<0.05),且观察组患者的治疗后8周及16周的膝关节伸屈最大角度均显著大于对照组(P<0.05)。结论改良外侧胫骨平台骨折塌陷关节面复位方法可以有效缩短患者的关节面塌陷高度,恢复其膝关节功能及活动度。Objective To analyze the clinical application effect of modified collapse articular surface reset on lateral tibial plateau frac-ture. Methods Patients with tibial plateau fracture received surgery from October 2012 to October 2013 were enrolled as study subject and ran-domly divided into control group receiving conventional treatment and observation group receiving modified collapse articular surface reset. Then col-lapse articular surface height ,knee score ,knee activity were detected before and after treatment. Results ①Shortly after surgery and the last time of follow - up,the collapse articular surface height of the two groups were lower than before surgery( P ﹤ 0. 05),and collapse articular sur-face height of observation group were lower than control group shortly after surgery and the last time of follow - up( P ﹤ 0. 05);②16 weeks after treatment,knee function scores of both two groups were significantly higher than those of 8 weeks after treatment( P ﹤ 0. 05);8 and 16 weeks after treatment,knee function scores of observation group were significantly higher than control group( P ﹤ 0. 05);③16 weeks after treatment, maximum knee activity of both two groups were significantly greater than those of 8 weeks after treatment( P ﹤ 0. 05);8 and 16 weeks after treat-ment,maximum angle of knee flexion of observation group were significantly greater than control group( P ﹤ 0. 05). Conclusion Modified col-lapse articular surface reset on lateral tibial plateau fracture can effectively reduce the height of articular surface collapse and restore knee function and activity.
