摘  要:目的探讨面肌痉挛显微血管减压术的治疗效果及预后情况。方法回顾性分析2(]04年1月至2012年1月河北省沧州市中心医院神经外科采用显微血管减压术治疗833例面肌痉挛患者的临床资料,根据患者的术后恢复情况分为即刻治愈、延迟治愈、复发和未愈4组,统计分析4组病例的临床数据。结果833例面肌痉挛行显微血管减压术的患者中,即刻治愈者472例,延迟治愈者302例,复发者33例,未愈者26例。术后1年手术治疗的有效率与2年有效率相差甚微。延迟治愈组的平均病史为(98.1±10.4)个月,较即刻治愈组的平均病史(26.3±5.6)个月明显延长,两组比较差异有统计学意义(P〈0.05)。结论显微血管减压术是治疗面肌痉挛的有效方法,显微血管减压术后1年是对手术效果判定较合适的期限,较长的病史可能会增加术后延迟治愈的可能性。Objective To investigate the efficacy and prognosis of hemifacial spasm microvascular decompression. Methods The clinical data of 833 patients with hemifacial spasm treated with microvascular decompression at the Department of Neurosurgery, Cangzhou Central Hospital, Hebei Province from January 2004 to January 2012 were analyzed retrospectively. According to the recovery of the patients after surgery, they were divided into 4 groups : instantly cure, delayed cure, recun'ent, and unhealed. The clinical data of 4 groups of patients were analyzed statistically. Results Of the 833 patients with hemifacial spasm underwent microvascular decompression, 472 were cured instantly, 302 were cured late; 33 had recurrence; and 26 were unhealed. The difference of the effective rate between 1 year and two years after surgery was minimal. The mean history (98. 1 ± 10. 4 months ) of the delayed cure group was significantly longer than the instantly cure group. There was significant difference between the two groups (P 〈 0. 05). Conclusions Microvascular decompression is an effective treatment method for hemifacial spasm. One year after microvascular decompression is the more appropriate time limit for identifying the operation effect. The longer medical history may increase the likelihood of delayed cure after procedure.
